摘 要:目的 探究在强直性脊柱炎骶髂关节炎骨髓水肿中应用双能量CT虚拟去钙技术的临床价值。方法于2023年6月至2024年5月,选取我院50例强直性脊柱炎骶髂关节炎患者,依据BASDAI评分进行分组,分为早期急性期(平均分≥6.0分)、慢性活动期(平均分在4.0-6.0分之间)各25例,均进行双能量CT虚拟去钙技术预测,对比其预测价值,并进行分析。结果 两组在年龄、病程及SPARCC评分中无差异(P>0.05),且早期急性期ESR、CRP低于慢性活动期(P<0.05)。骨髓水肿SPARCC评分与病情活动性BASDAI评分呈正相关(rs=0.682,P<0.05)。早期急性期VNCa CT、有效原子序数、能谱曲线斜率及rCT值高于慢性活动期(P<0.05)。本次研究以MRI图像显示骨髓水肿区域为“金标准”发现,使用双能量CT虚拟去钙技术检测的阳性检出率较低(P<0.05)。双能量CT虚拟去钙技术灵敏度、特异度和准确度、AUC值均较高(P<0.05)。结论双能量CT虚拟去钙技术应用于强直性脊柱炎骶髂关节炎骨髓水肿具有较高的准确率,且诊断效能较高。Objective To explore the clinical value of dual energy CT virtual decalcification technology in the treatment of bone marrow edema in ankylosing spondylitis sacroiliac arthritis.Methods From June2023 to May 2024,50 patients with a n kylosing spondylitis sacroiliitis in our hospital were selected and divided into early acute phase(average score≥6.0 points)and chronic active phase(average score between 4.0-6.0 points)based on the BASDAI score.Each patient was predicted using dual energy CT virtual decalcification technology,and their predictive value was compa red and analyzed.Results There was no difference between the two groups in age,disease duration,ESR,CRP,and SPARCC scores(P>0.05).The SPARCC score of bone marrow edema is positively correlated with the BASDAI score of disease a ctivity(rs=0.682,P<0.05).In the early acute phase,VNCa CT,effective atomic number,energy spectrum curve slope,and rCT values were higher than those in the chronic active phase(P<0.05).This study used MRI images to display areas of bone marrow edema as the"gold standard"and found that the positive detection rate using dual energy CT virtual decalcification technology was relatively low(P<0.05).The dual energy CT virtual calcium removal technology has high sensitivity,specificity,accuracy,and AUC values(P<0.05).Conclusion The application of dual energy CT virtual decalcification technology in ankylosing spondylitis sacroiliac joint bone marrow edema has high accuracy and diagnostic efficacy.
