预警征象筛查在社区儿童健康管理中的应用

摘　　要：目的了解北京市朝阳区潘家园社区0~3岁儿童心理行为发育水平,探寻儿童心理行为在社区的筛查模式。方法使用儿童心理行为发育问题预警征象筛查表对2023年1—12月在潘家园社区接受健康检查的2139名0~3岁儿童进行心理行为发育预警征象的筛查,统计分析儿童相应年龄的发育偏异情况,并对筛查阳性儿童进行单、多因素分析。结果在2139名儿童中,儿童心理行为发育问题预警征阳性的儿童70例,阳性率为3.27%;男童心理行为发育预警征阳性率为4.38%,女童阳性率为2.06%,两者比较,差异有统计学意义(P<0.05)。<1岁组儿童心理行为发育问题预警征阳性率为0.56%,≥1岁且<2岁组儿童阳性率为2.38%,≥2岁且<3岁组儿童阳性率为6.86%,≥3岁且<4岁组儿童阳性率为2.58%,不同年龄组儿童的阳性率差异有统计学意义(P<0.05)。多因素Logistic回归分析显示,性别、年龄是儿童心理行为发育预警征筛查阳性的危险因素。结论预警征象筛查在社区基层儿童的健康管理中具有较好的应用价值,能够早期发现心理行为发育偏异的儿童,以便对其进行有效干预,促进儿童早期全面健康发展。Objective To learn the psychological and behavioral development level of children aged 0-3 in Panjiayuan Community,Chaoyang District,Beijing City,and explore the screening model of children's psychological behavior in the community.Methods The Screening Form for Warning Signs of Children's Psychological and Behavioral Development Problems was used to screen 2139 children aged 0-3 who received health check-up in Panjiayuan Community from January to December 2023.The developmental deviations of the children at corresponding ages were statistically analyzed,and single and multiple factor analyses were conducted on the screened positive children.Results Among 2139 children,70 had positive warning signs for psychological and behavioral development problems,with a positive rate of 3.27%.The positive rate of psychological and behavioral development warning signs in boys was 4.38%,while in girls it was 2.06%.The difference between the two was statistically significant(P<0.05).The positive rate of warning signs for psychological and behavioral development problems in<1 year children is 0.56%,the positive rate in children aged≥1 and<2 years is 2.38%,the positive rate in children aged≥2 and<3 years is 6.86%,and the positive rate in children aged≥3 and<4 years is 2.58%.There is a statistically significant difference in the positive rate among children of different age groups(P<0.05).Multivariate logistic regression analysis showed that gender and age are risk factors for positive screening of warning signs in children's psychological and behavioral development.Conclusion The screening of warning signs has good application value in the health management of grassroots children in the community.It can detect children with psychological and behavioral developmental deviations early,so as to effectively intervene and promote their comprehensive and healthy development in early childhood.
