黏膜组织内Runx3蛋白、HIF-1α表达水平与不同类型胃息肉幽门螺杆菌感染的关系分析

摘  要:目的 探讨黏膜组织内Runt相关转录因子3(runtrelated transcription factor 3,Runx3)和缺氧诱导因子-1α(hypoxia-Inducible Factor 1-Alpha,HIF-1α)蛋白表达水平与不同类型胃息肉及幽门螺杆菌(H.pylori,Hp)感染的关系。方法 选取2018年1月至2022年1月在本院诊治的胃息肉患者154例为病例A组,慢性萎缩性胃炎患者120例为病例B组,胃癌患者120例为病例C组。通过胃黏膜活检确诊,检测各组Runx3、HIF-1α蛋白和Hp阳性表达率,并分析胃息肉患者中Runx3、HIF-1α蛋白与Hp感染的相关性。结果 Runx3、HIF-1α蛋白在A、B、C组中的阳性表达率分别为62.99%vs47.40%、75.33%vs33.33%、31.67%vs85.83%(P<0.001)。增生性息肉组与病例C组的Runx3、HIF-1α蛋白阳性率差异显著(P<0.001),与病例B组无显著差异(P>0.05);腺瘤性息肉组与病例C组无显著差异(P>0.05),与病例B组有差异(P<0.05)。Hp感染阳性率在A、B、C组分别为45.45%、40.83%、84.17%(P<0.001)。增生性息肉组与病例C组差异显著(P<0.001),与病例B组无差异(P>0.05);腺瘤性息肉组与病例C组无差异(P>0.05),与病例B组差异显著(P<0.05)。Hp感染阳性患者中Runx3和HIF-1α蛋白阳性率分别为24.29%和52.86%;Hp感染阴性患者中分别为95.24%和42.86%。相关性分析显示,胃息肉患者的Runx3表达与Hp感染呈负相关(r=-0.732,P<0.001),HIF-1α与Hp感染无显著相关性(r=0.100,P=0.213)。结论 Runx3和HIF-1α蛋白在胃息肉患者黏膜组织中呈高阳性表达,Hp感染可能参与胃息肉发生发展,其中Runx3与Hp感染呈负相关性。Objective To investigate the relationship between the expression levels of runt-related transcription factor 3(runtrelated transcription factor 3,Runx3)and hypoxia-inducible factor-1α(hypoxia-Inducible Factor 1-Alpha,HIF-1α)proteins in mucosal tissues and different types of gastric polyps,as well as their association with Helicobacter pylori(H.pylori,Hp)infection.Methods A total of 154 patients with gastric polyps treated at our hospital from January 2018 to January 2022 were included as Group A.Additionally,120 patients with chronic atrophic gastritis were included as Group B,and 120 patients with gastric cancer as Group C.Diagnoses were confirmed via gastric mucosal biopsy.The expression levels of Runx3 and HIF-1αproteins and the Hp positivity rates in all groups were assessed.Correlations between Runx3,HIF-1αproteins,and Hp infection in patients with gastric polyps were analyzed.Results The positive expression rates of Runx3 and HIF-1αin Groups A,B,and C were 62.99%vs.47.40%,75.33%vs.33.33%,and 31.67%vs.85.83%,respectively(P<0.001).Significant differences were found in Runx3 and HIF-1αpositivity rates between the hyperplastic polyp subgroup and Group C(P<0.001),while no significant differences were observed between the hyperplastic polyp subgroup and Group B(P>0.05).Conversely,no significant differences were found between the adenomatous polyp subgroup and Group C(P>0.05),but significant differences existed between the adenomatous polyp subgroup and Group B(P<0.05).Hp positivity rates in Groups A,B,and C were 45.45%,40.83%,and 84.17%,respectively(P<0.001).Significant differences in Hp positivity were observed between the hyperplastic polyp subgroup and Group C(P<0.001)but not between the hyperplastic polyp subgroup and Group B(P>0.05).Similarly,no differences were observed between the adenomatous polyp subgroup and Group C(P>0.05),while differences existed between the adenomatous polyp subgroup and Group B(P<0.05).Among Hp-positive patients,the positive expression rates of Runx3 and HIF-1αwere 24.29%and
