摘 要:目的探究孕中期应用四维超声时间-空间相关成像(STIC)技术联合二维超声筛查胎儿心脏畸形的可行性。方法选取2021年1月至2023年1月86例于本院进行产检经二维超声筛查高度怀疑胎心畸形孕中期孕妇作为研究对象进行回顾性分析,以出生或引产后胎儿情况为“金标准”,分析STIC技术联合二维超声诊断效能。结果86例胎儿引产终止妊娠77例,继续妊娠9例;出生或引产后胎儿情况结果证实心脏畸形83例,其中室间隔缺损28例,血管环27例,大血管转位10例,永存动脉干9例,单心房/室6例,法洛四联症3例。STIC技术联合二维超声筛查心脏畸形的准确度为98%(84/86),灵敏度为98%(81/83),特异度为100%(3/3)。STIC技术联合二维超声筛查心脏畸形类型的准确度与金标准差异无统计学意义P>0.05。结论STIC技术联合二维超声用于孕中期胎儿心脏畸形筛查可行性强,解剖信息全面且准确,建议临床推广使用。Objective To explore the feasibility of four-dimensional ultrasound spatio-temporal image correlation(STIC)combined with two-dimensional ultrasound in the screening of fetal heart malformations in the second trimester of pregnancy.Methods Eight six pregnant women in the second trimester of pregnancy who were highly suspected of fetal heart malformation by two-dimensional ultrasound screening in our hospital were retrospectively analyzed from January 2021 to January 2023.With the fetal condition after birth or induction of labor is the gold standard for analyzing the diagnostic efficiency of STIC technology combined with two-dimensional ultrasound.Results Among 86 pregnant women,77 cases performed fetal induced labor and terminated pregnancy,and 9 cases continued to pregnancy.Fetal conditions after birth or induction of labor confirmed cardiac malformations in 83 cases,including 28 cases of ventricular septal defects,27 cases of vascular ring,10 cases of transposition of great vessels,9 cases of persistent truncus arteriosus,6 cases of single atrium/ventricle and 3 cases of tetralogy of Fallot.The accuracy,sensitivity,and specificity of STIC technology combined with two-dimensional ultrasound screening for cardiac malformation were 98%(84/86),98%(81/83),and 100%(3/3).The accuracy of STIC technology combined with two-dimensional ultrasound for screening cardiac malformation types is close to the gold standard(P>0.05).Conclusion STIC technology combined with two-dimensional ultrasound has strong feasibility in the screening of fetal heart malformations in the second trimester of pregnancy,and the anatomical information is comprehensive and accurate.
