摘　　要：目的观察相关指南内中国专家Ⅰ级推荐行^(18)F-FDG PET/CT检查淋巴瘤的PET/CT代谢特点。方法回顾性分析115例接受基线^(18)F-FDG PET/CT检查的淋巴瘤患者,根据指南推荐级别分为Ⅰ级A组[n=69,国际专家Ⅰ级推荐淋巴瘤,包括霍奇金淋巴瘤(HL)及弥漫大B细胞淋巴瘤(DLBCL)],Ⅰ级B组[n=26,中国专家Ⅰ级推荐淋巴瘤,包括外周T细胞淋巴瘤(PTCL)、纵隔大B细胞淋巴瘤(PMBL)、伯基特淋巴瘤(BL)、高级别滤泡性淋巴瘤(FL)及淋巴母细胞白血病/淋巴瘤]及Ⅱ级组[n=20,国际专家Ⅱ级推荐淋巴瘤,包括结边缘区淋巴瘤(MZL)、套细胞淋巴瘤(MCL)、低级别FL及非胃黏膜相关淋巴瘤(MALToma)];比较3组及两两组间PET/CT参数。结果3组肿瘤最大标准摄取值(SUV_(max))、平均标准摄取值、峰值标准摄取值、最大瘦体质量标准摄取值、平均瘦体质量标准摄取值、峰值瘦体质量标准摄取值、病灶/肝脏SUV_(max)及病灶/纵隔SUV_(max)差异均有统计学意义(P均<0.05);其中,Ⅱ级组与其他2组上述参数差异均有统计学意义(P均<0.05)。结论指南内中国专家Ⅰ级推荐行^(18)F-FDG PET/CT检查淋巴瘤呈高摄取,与HL及DLBCL类似而高于MZL、MCL、低级别FL和非胃MALToma。Objective To observe the metabolic characteristics of lymphomas with Chinese experts gradeⅠrecommendation for examination of^(18)F-FDG PET/CT guideline.Methods Totally 115 patients with lymphoma who underwent baseline^(18)F-FDG PET/CT were retrospectively enrolled and divided into gradeⅠA group(n=69,lymphomas with international experts gradeⅠrecommendation,including Hodgkin lymphoma[HL]and diffuse large big cell lymphoma[DLBCL]),gradeⅠB group(n=26,lymphomas with Chinese experts gradeⅠrecommendation,including peripheral T-cell lymphoma[PTCL],primary mediastinal large B-cell lymphoma[PMBL],Burkitt lymphoma[BL],high-grade follicular lymphoma[FL]and lymphoblastic leukemia/lymphoma)and gradeⅡgroup(n=20,lymphomas with international experts gradeⅡrecommendation,including marginal zone lymphoma[MZL],mantle cell lymphoma[MCL],low-grade FL and non-gastric mucosa-associated lymphoma tissue lymphoma[MALToma]).PET/CT parameters were compared among 3 groups and between each 2 groups.Results Significant differences of the maximum standard uptake value(SUV_(max)),the mean standard uptake value,peak standard uptake value,the maximum standard uptake value normalized by lean body mass,the mean standard uptake value normalized by lean body mass,peak standard uptake value normalized by lean body mass,lesion/liver SUV_(max)and lesions/mediastinum SUV_(max)were found among 3 groups(all P<0.05),also between gradeⅡgroup and each of the other 2 groups(both P<0.05).Conclusion Lymphomas with Chinese experts gradeⅠrecommendation for examination of^(18)F-FDG PET/CT guideline presented high metabolism on^(18)F-FDG PET/CT,similar to HL and DLBCL but higher than MZL,MCL,low-grade FL and non-gastric MALToma.
