血府逐瘀汤加减治疗气滞血瘀型躯体化障碍的临床研究

摘　　要：目的:探讨血府逐瘀汤加减治疗气滞血瘀型躯体化障碍患者的临床效果。方法:将医院门诊治疗的66例气滞血瘀型躯体化障碍患者随机分为两组,各33例患者。对照组患者予以舒肝解郁胶囊联合谷维素治疗,研究组患者予以血府逐瘀汤加减治疗,每组连续治疗2周。比较两组治疗前后临床症状、汉密尔顿抑郁量表(HAMD)评分、汉密尔顿焦虑量表(HAMA)评分、症状自评量表(SCL-90)评分,以及治疗后第3、9、12个月的复发率,分别评价两组的临床疗效及安全性。结果:治疗后,两组患者的临床症状均减轻,且研究组患者临床治疗总有效率高于对照组(P<0.05);两组患者HAMA评分、HAMD评分、SCL-90躯体化因子评分均降低,且研究组低于对照组(P<0.05);研究组患者在治疗1年后的复发率低于对照组(P<0.05)。结论:对气滞血瘀型躯体化障碍患者予以血府逐瘀汤加减治疗能够显著缓解患者各临床症状,并提升整体效果,降低复发率。Objective:To observe the clinical application effect of modified Xuefu Zhuyu Decoction in the treatment of patients with somatoform disorders with Qi stagnation and blood stasis syndrome.Methods:66 patients with somatoform disorders with Qi stagnation and blood stasis syndrome treated at the outpatient clinic of Yingshang County Traditional Chinese Medicine Hospital were randomly divided into two groups,with 33 patients in each group.The control group was treated with Shugan Jieyu Capsules combined with Oryzanol,while the study group was treated with modified Xuefu Zhuyu Decoction.Each group was treated continuously for 2 weeks.Clinical symptoms,Hamilton Depression Rating Scale(HAMD)scores,Hamilton Anxiety Rating Scale(HAMA)scores,Symptom Checklist-90(SCL-90)scores before and after treatment,and recurrence rates at the 3rd,9th,and 12th months after treatment were compared between the two groups to evaluate the clinical efficacy and safety of the two groups,respectively.Results:After treatment,the clinical symptoms of patients in both groups were alleviated,and the clinical total effective rate in the study group was higher than that in the control group(P<0.05).The HAMA scores,HAMD scores,and SCL-90 somatization factor scores decreased in both groups,and those in the study group were lower than those in the control group(P<0.05).Compared with the control group,the recurrence rate in the study group was lower one year after treatment(P<0.05).Conclusion:Modified Xuefu Zhuyu Decoction for patients with somatoform disorders with Qi stagnation and blood stasis syndrome can significantly alleviate various clinical symptoms,improve the overall effect,and minimize recurrence.
