超声下髂筋膜阻滞对老年股骨粗隆间骨折患者的影响

摘　　要：目的:评估超声下髂筋膜阻滞对老年股骨粗隆间骨折患者的影响。方法:选取2022年6月—2023年12月青海红十字医院收治的96例老年股骨粗隆间骨折患者作为研究对象,随机分成对照组和研究组,每组48例。对照组采用腰硬联合麻醉,研究组采用超声下髂筋膜阻滞。比较两组血流动力学指标、疼痛情况及直腿抬高能力。结果:麻醉前,两组平均动脉压、心率水平比较,差异无统计学意义(P>0.05);麻醉诱导后5 min、切皮时、术毕,两组平均动脉压、心率水平低于麻醉前,研究组高于对照组(P<0.05)。术后4、8、12、24 h,研究组视觉模拟评分法评分低于对照组(P<0.001)。术后6 h、24 h、3 d、7 d,两组直腿抬高能力对比,差异无统计学意义(P>0.05)。结论:超声下髂筋膜阻滞能够改善老年股骨粗隆间骨折手术的血流动力学水平,减轻术后疼痛,利于患者康复。Objective:To evaluate the effect of iliac fascia block under ultrasound on elderly patients with intertrochanteric fracture of femur.Methods:A total of ninety-six elderly patients with intertrochanteric fracture of femur treated in Qinghai Red Cross Hospital from June 2022 to December 2023 were selected as the study objects and randomly divided into control group and study group,with forty-eight cases in each group.The control group received combined epidural anesthesia,and the study group received iliac fascia block under ultrasound.Hemodynamic parameters,pain and straight leg elevation ability were compared between the two groups.Results:Before anesthesia,there was no significant difference in mean arterial pressure and heart rate between the two groups(P>0.05);5 min after anesthesia induction,at the time of skin incision and after surgery,the mean arterial pressure and heart rate of the two groups were lower than before anesthesia,and the study group was higher than the control group(P<0.05).At 4,8,12 and 24 h after operation,the Visual Analogue Score of the study group was lower than that of the control group(P<0.001).At 6 h,24 h,3 d and 7 d after surgery,there was no significant difference in straight leg elevation ability between the two groups(P>0.05).Conclusion:Iliac fascia block under ultrasound can improve the hemodynamic level of elderly femoral intertrochanteric fracture,reduce postoperative pain,and facilitate the rehabilitation of patients.
