摘 要:目的:研究肥胖儿童体质量指数(BMI)与动脉粥样硬化相关指标的相关性。方法:选取2021年6月—2022年6月于茂名市人民医院进行健康体检的儿童300例作为研究对象,根据BMI分为正常组(n=130)、超重组(n=80)、肥胖组(n=90)。比较三组生化指标[胰岛素敏感指数(HOMA-IS)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、C反应蛋白(CRP)、可溶性细胞间黏附因子-1(sICAM-1)、脂联素(APN)]、颈动脉内膜中层厚度(IMT)与颈动脉弹性水平,分析儿童BMI与生化指标、IMT与颈动脉弹性指标的相关性。结果:相较超重组、正常组,肥胖组HOMA-IS、HDL-C、APN及颈动脉弹性更低,TG、LDL-C、CRP、sICAM-1、IMT数值更高,差异有统计学意义(P<0.05)。儿童BMI与TG、LDL-C、CRP、sICAM-1、IMT呈正相关,与HOMA-IS、HDL-C、APN、颈动脉弹性呈负相关(P<0.001)。结论:BMI过高儿童面临更高的糖脂代谢异常、内皮功能损伤、动脉粥样硬化风险。Objective:To study the correlation between body mass index(BMI)and atherosclerosis related indexes in obese children.Methods:A total of three hundred children who underwent physical examination in Maoming People's Hospital from June 2021 to June 2022 were selected as the study objects,and were divided into normal group(n=130),superrecombinant group(n=80)and obese group(n=90)according to BMI.Biochemical indicators[insulin sensitivity index(HOMA-IS),triglycerides(TG),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),C-reactive protein(CRP),soluble intercellular adhesion molecule-1(sICAM-1),adiponectin(APN)],intima-media thickness(IMT)and carotid artery elasticity levels were compared between three groups.The correlation between BMI and biochemical indexes,IMT and carotid artery elasticity indexes were analyzed.Results:Compared with superrecombinant and normal groups,HOMA-IS,HDL-C,APN and carotid artery elasticity in obese group were lower,and TG,LDL-C,CRP,sICAM-1 and IMT values were higher,the difference was statistically significant(P<0.05).BMI in children was positively correlated with TG,LDL-C,CRP,sICAM-1 and IMT,and negatively correlated with HOMA-IS,HDL-C,APN and carotid artery elasticity(P<0.001).Conclusion:Children with high BMI face higher risk of abnormal glucose and lipid metabolism,endothelial function impairment and atherosclerosis.
