摘  要:目的 :探讨脑干听觉诱发电位 (BAEP)对前庭周围性眩晕和中枢性眩晕的鉴别诊断价值。方法 :对 86例前庭系统性眩晕患者进行BAEP检查 ,观察Ⅰ、Ⅲ、Ⅴ波的潜伏期 (PL)、波形分化情况和Ⅰ—Ⅲ、Ⅲ—Ⅴ、Ⅰ—Ⅴ波的峰间期潜伏期 (IPL)、两耳测间差 (ILD)以及Ⅰ /Ⅴ波幅比值。结果 :86例中有 5 1例 (5 9% )异常 ,依其表现分为两型 :①内耳型 2 4例 :表现为Ⅰ波分化不良或波形消失 ,PL延长及其后各波相应延长 ,ILD相应增大 ;②脑干型 2 7例 :表现为Ⅰ波正常 ,Ⅲ、Ⅴ波分化不良或波形消失 ,Ⅲ波或Ⅴ波PL、Ⅰ—Ⅲ及Ⅲ—ⅤIPL延长 ,ILD相应增大。结论Objective:To explore the diagnostic value of brainstem auditory evoked potential in patients with vestibular system vertigo.Method:BAEP was detected in 86 patients with vestibular system vertigo.The peak latency(PL) and change of wave Ⅰ?Ⅲ?Ⅴ, the interpeak latency(IPL) of wave Ⅰ-Ⅲ?Ⅲ-V?and?Ⅰ-Ⅴ, the interear latency difference(ILD) of two ears and the ratio of Ⅰ/Ⅴwere observed.Results:BAEP was abnormal in 51 cases.The abnormality rate was 59%(51/86).Two types of abnormality were classified according to their characteristics,as follows: ①inner-ear type (24 cases): showing undifferentiation or disappearance of wave I, prolongation of PL of wave I and all later waves ,increace of IDL.②Brainstem type (27 cases):showing normal wave I,undifferentiation or disappearance of wave Ⅲ and wave Ⅴ,prolongation of PL of wave Ⅲ and wave Ⅴ,also prolongation of IPL of waveⅠ-Ⅲ and Ⅲ-Ⅴ,as well as increace of ILD.Conclusion:BAEP detection might be a more sensitive vestibular peripheral and central vertigo.
