血清透明质酸等的变化与COPD肺纤维化的关系

摘　　要：目的研究慢性阻塞性肺疾病患者血清细胞外基质指标变化的临床意义。方法检测33例慢性阻塞性肺疾病患者血清透明质酸(HA)、层粘连蛋白(LN)、Ⅲ型前胶原(PCⅢ)、Ⅳ型胶原(CⅣ),与对照组进行比较。并分析与疾病严重程度及炎症指标的关系。结果慢性阻塞性肺疾病患者血清HA(165.6±205.5)ng/ml(P<0.05),LN(98.8±14.7)ng/ml(P>0.05),PCⅢ(139.0±54.2)ng/ml(P<0.01),CⅣ(72.6±34.8)ng/ml(P<0.01);对照组分别为HA(69.6±19.4)ng/ml,LN(96.7±19.8)ng/ml,PCⅢ(97.3±26.6)ng/ml,CⅣ(48.3±18.4)ng/ml。肺心病并呼吸衰竭与慢性阻塞性肺疾病无呼吸衰竭各组相比HA明显增高(P<0.05),HA、CⅣ与炎症指标呈正相关(P<0.05)。结论对慢性阻塞性肺疾病患者联合检测HA、LN、PCⅢ、CⅣ,对本病肺纤维化的诊断、活动性的观察及研究肺纤维化的发病机理具有重要价值。Objective To investigate serum extracellular matrix parameters levels in chronic obstructive pulmonary disease and probe into their significance.Methods The serum hyaluronic acid(HA),laminin(LN),procollagenⅢ(PCⅢ)and collagenⅣ(CⅣ)were detected in33cases with chronic obstructive pulmonary disease.The results were compared with that of normal control group.The relationship between severity of disease and inflammation markers was analyzed.Results The results of serum HA,LN,PCⅢand CI V were(165.6±205.5)ng/ml (P<0.05),(98.8±14.7)ng/ml (P>0.05),(139.0±54.2)ng/ml (P<0.01),(72.6±34.8)ng/ml (P<0.01)in chronic obstructive pulmonary disease patient,and were(69.6±19.4)ng/ml,(96.7±19.8)ng/ml,(97.3±26.6)ng/ml,(48.3±18.4)ng/ml in normal control group respectively.The serum HA significantly higher in group of pulmonary heart disease with respiration failure than other groups of chronic obstructive pulmonary disease without respiration failure (P<0.05).Serum HA,CⅣwas positively correlated with inflammation markers.Conclusion It is important valuable for diagnosis of pulmonary fibrosis,observation of its activity,and its mechanism investigation to detect serum HA,LN,PCⅢ,CⅣin chronic obstructive pulmonary disease.
