应用Amplatzer封堵器经皮穿刺治疗房间隔缺损及动脉导管未闭(附8例报告)

摘　　要：目的　报告讨论应用Amplatzer封堵器治疗二孔型房间隔缺损 (ASD)及动脉导管未闭 (PDA)的经验。材料方法　全组 8例 ,ASD4例 ,PDA4例 ;年龄 8-64岁 (平均 3 2 .4± 17.2岁 ) ,体重 18-75kg(平均 60 .0± 14 .8Kg)。术前均常规经胸超声心动图 (TTE)确认ASD或PDA位置、大小及形态 ;分流情况及肺动脉压。在透视监视下经导管置入封堵器。成人ASD术中在经食道超声 (TEE)监视下了解ASD的诸边缘及封堵结果 ;儿童使用经胸超声监视。PDA封堵前后行主动脉造影 ,确认PDA形态、大小及封堵结果。术后 1个月及 6个月分别经TTE评价治疗结果。结果　全组 7例手术成功率 10 0 % ,无任何并发症。术后 2 4小时TTE显示全部病例封堵成功 ,无残余分流。结论　经皮穿刺应用Amplatzer封堵器治疗二孔型ASD及PDA疗效肯定 ,操作简便 ,成功率高 。Purpose: To report the experience of transcatheter closure of secundum atrial septal defect(ASD) and patent ductus arteriosus(PDA) using Amplatzer occluder device. Materias and methods: There was 8 cases. ASD was 4 cases,PDA was 4 cases.The age was from 8 to 64 years old(mean age 32.4±17.2 years). Transthoracic echocardiograms were performend in pre-operation for each caeses.It is confimed that status about ASD and PDA, include the diameter and site of defect and pulmonary prassure. Each cases was occluded using Amplatzer occluder device throught the percutaneous procedure. The closure procedure was guided by fluoroscopy. ASD cases the procedure was monitoried by transesophageal echocardiography, childe case was by transthoracic echocardiography(TTE).Aorta angiography were carried out in pre and post-procedure. Total cases were evaluated using TTE at 1 month and 6 months after operation. Results: The successful rate of operation was 100%.The residual shunt would not found in total case after operation 24 hours. Conclusion: Transcatheter closure of ASD and PDA using Amplatzer occluder was an efficient interventional method.It had a high successful rate of operation and the procedure simple.
