动态监测缺氧缺血性脑病患儿血清CK-BB的临床意义

摘　　要：目的　探讨缺氧缺血性脑病 (HIE)新生儿血清脑型肌酸激酶同工酶 (CK- BB)和心型肌酸激酶同工酶 (CK-MB)的活性变化 ,以了解 CK- BB和 CK- MB对 HIE患儿的检测价值。方法　在出生后 1～ 3d检测 HIE患儿 CK、CK-BB、CK- MB活性 ,与正常新生儿作对比 ,分析各项指标与病情、转归、预后的关系。结果　中度、重度 HIE患儿血清 CK-BB、CK与正常新生儿相比有显著性升高 ;重度组 CK- MB比正常组显著升高。 CK- BB活性迅速下降者 ,治疗有效 ,预后良好 ;CK- BB活力持续升高 ,患儿预后差 ,CK- MB升高患儿常伴有心肌功能损害。结论　血清 CK- BB是诊断 HIE的可靠、特异性指标 ,动态监测新生儿 CK- BB、CK- MB活性对病情估计、判断疗效及预后评估有重要价值。Objective To explore the changes of serum creatine kinase BB,CK,CK MB activites in neonates with hypoxic ischemic encephalopathy(HIE) and to find out the activities of CK BB, CK MB in serum in HIE.Methods The sera CK BB,CK, CK MB activities of 1～3 days old neonates were examined dynamically in 48 HIE neonates and normal neonates.The relationships between these enzymes' activities and state of illness and its prognosis.Results The activities of sera CK ,CK BB in moderate and severe HIE were significantly higher than those in control groups (P<0.05 ), CK MB were evidently higher in severe HIE than in control groups (P<0.05 ) . The rapid decrease and recovery of CK BB activity was the symbol of successful treatment and eusemia, while the increase of CK MB activity was often companied with cardiac muscle injury.Conlusion The serum CK BB was a reliable,specific index , and dynamical monitoring serum CK BB,CK MB activities was very useful in the evaluation of treatment effects,cerebral ischemia and anoxia degree and prognosis.
