摘 要:目的 比较神经不同节段扩张延长与神经移植修复神经缺损的效果。方法 将SD大鼠按手术先后分成 5组 ,每组 8只大鼠。A组近段扩张 ,B组远段扩张 ,C组近远段同时扩张 ,D组移植 ,E组为对照组。扩张后分别进行神经修复。术后 60~ 80d作组织学、电生理学、计算机图像分析及腓肠肌肌湿重检测。结果 A、B与C组的坐骨神经约延长 30 %。各项检测结果均显示以C组的神经再生最好 ,以下依次为A、B、D组。结论 用组织扩张器延长周围神经可修复长段神经缺损 ,效果优于自体神经移植。神经近、远段同时扩张延长后修复神经缺损的效果优于近段扩张或远段扩张 。Objective To compare the treatment outcome for repair of peripheral nerve defects between by expansion of different segment of nerve and by nerve grafting. Methods Forty SD rats were divided into 5 groups, with 8 in each, that is,expansion of proximal nerve(group A), expansion of distal segment(group B), expansion of both proximal and distal parts(group C), grafting (group D) and control group (group E). Nerve repair was done after expansion. On postoperative 60 to 80 days, histological examinations, neurophysiological testing and computer image analysis of the nerve, and gastrocnemius muscle wet weight test were carried out.Results About 30% elongation of the sciatic nerve was achieved in group A,B and C. The best regeneration of nerve was observed in group C by all parameters, decreasing in order of group A, B and D. Conclusions Nerve stretch by tissue expander could be used to repair long nerve defect, and its result was superior to that of nerve autografting. The result obtained by both proximal and distal elongation was better than that by proximal or distal elongation, and that by proximal elongation was better than that by distal elongation.
