摘  要:目的 观察高血压病人颈动脉结构及功能的变化 ,并进一步探讨肱动脉脉压与颈动脉结构及功能改变是否相关。方法 选取原发性高血压病人 5 4例 (EH组 ) ,健康自愿者 4 7例 (NC组 )作为观察对象。应用二维超声测定颈动脉结构与功能。结果 EH组与NC组间年龄、体重指数、性别构成无差异 (P >0 0 5 )。EH组的收缩压、舒张压、脉压、平均动脉压、颈动脉内膜—中层厚度 (IMT)、僵硬度较NC组显著升高 (P均 <0 0 5 ) ,EH组的颈动脉扩张性较NC组显著降低 (P <0 0 5 )。直线相关分析显示 ,年龄、收缩压、脉压与高血压病人颈动脉IMT、内径密切相关 ,年龄、收缩压、舒张压、脉压与高血压病人颈动脉扩张性、僵硬度密切相关 ,而且高血压病人颈动脉内膜—中层厚度、内径、扩张性、僵硬度各参数间也密切相关。结论 高血压病人颈动脉内膜—中层厚度增厚、扩张性降低、僵硬度增加 。Objective To investigate the changes of carotid structure and function, and evaluate the relations between pulse pressure and the structure and function of carotid artery in hypertensive patients. Methods Fifty four essential hypertensive patients (EH group) and forty seven healthy people(normal control (NC group) were enrolled. Structure and function of carotid were assessed by high resolution B mode ultrasonography. Results SBP,DBP, pulse pressure(PP), mean arterial pressure(MAP), carotid intima media thickness(IMT) and stiffness(S) were higher in EH group than in NC group ( P <0 05),while carotid distensibility(CD) was lower ( P <0 05). Linear correlation analysis demonstrated that age, SBP, PP were closely related to carotid IMT and internal diameter(ID), and age, SBP, DBP, PP were related to CD and S, carotid IMT, ID, CD and S were closely related each other in hypertensive patients. Conclusion In hypertensive patients, carotid IMT and S increase, and CD decreases. The distensibility and stiff of carotid artery are closely rolated to PP in brachial artery.
