摘 要:目的 探讨单层螺旋CT(SSCT)检测冠状动脉钙化的临床价值。方法 60例临床怀疑或确诊冠心病患者均行单层螺旋CT检查及冠脉造影。单层螺旋CT采用层厚 3mm ,Pitch =1,容积扫描。钙化的定义为CT值≥ 90Hu ,面积≥1mm2 。冠脉造影采用Judkins法 ,以存在血管狭窄≥ 5 0 %为诊断冠心病的标准。结果 60例中冠心病患者 3 8例 ,无冠心病患者 2 2例。冠心病组的平均钙化分数为 169.5 5± 2 2 7.5 3 ,明显高于非冠心病组的平均钙化分数 ( 3 .88± 11.2 4) (P <0 .0 1)。以单层螺旋CT检测有无冠脉钙化判断冠心病 ,在 5 0岁以下组 (年轻组 )敏感度为 43 % ,特异度为 93 %。在 5 0岁以上组 (老年组 )敏感度为 95 % ,特异度为 75 %。结论 单层螺旋CT检测冠脉钙化对诊断冠心病有重要的临床价值。Objective To evaluate the clinical value of coronary artery calcification detected by single slice helical CT (SSCT) . Methods Sixty patients suspected or confirmed with coronary artery diseases (CAD) were studied. All the patients underwent both SSCT examination and coronary angiography separately. SSCT scan was performed with thickness of 3mm and pitch=1. Calcification was defined as a density≥90Hu and area≥1mm 2. Coronary angiography was performed using Judkins technique. Criteria of CAD was vessel stenosis≥50% on angiogram. Results Of 60 cases, 38 cases were diagnosed as CAD by coronary angiography and other 22 were normal. There was significant difference ( P <0.01) in the mean coronary artery calcification (CAC) score between CAD group and non CAD group (169.55±227.53 vs 3.88±11.24). In the subgroup of age<50, the sensitivity of calcification for evaluating coronary stenosis was lower and the specificity was higher compared with the subgroup of age ≥50 (43% vs 95% and 93% vs 75% respectively). Conclusion The detection of CAC by SSCT is of much clinical relevance to CAD.
