中药健胃愈疡颗粒剂治疗大鼠实验性胃溃疡

摘　　要：目的:研究健胃愈疡颗粒治疗消化性溃疡的机制.方法:采用改良Okabe法复制大鼠胃溃疡模型.76只SD大鼠随机分为4组,除正常组外,其余三组均手术造模,每日灌胃分别饲以蒸馏水和不同药物,连续7d或14d后取溃疡胃组织,作常规病理检查,观察溃疡情况,TUNEL法检测细胞凋亡,免疫组化法检测各指标蛋白表达,原位杂交法法检测各指标mRNA表达.结果:JWYY治疗组在7d和14d后,溃疡指数(1.7±0.5,0.8±0.4)、凋亡指数(3.0±0.4,2.0±0.5)明显降低;MK蛋白(33.7±3.7,38.0±4.1)和mRNA(22.6±2.9,30.5±3.7)、EGFR蛋白(39.6±4.9,31.9±6.8)和mRNA(26.6±5.0,17.4±1.6)表达明显升高,较模型组和胃苏组有显著统计学差别(P<0.01或0.05).JWYY组7d时TNF-α蛋白(18.7±3.3)和mRNA(11.4±3.0)表达显著低于模型组(26.1±3.8,;17.1±2.7;P<0.01),而高于正常组(4.0±0.9,3.3±1.0,P<0.01);14d时各组则无明显统计学差别(P>0.05).JWYY组7d和14d时Bcl-2蛋白(33.5±6.8,32.9±9.6)、mRNA(26.1±4.5,23.0±6.2)及mRNABcl-2/Bax(1.4±0.3,1.5±0.4),蛋白Bcl-2/Bax(1.3±0.3,1.7±0.7)均显著高于模型组(P均<0.05或0.01).结论:JWYY颗粒通过提高Bcl-2、Bcl-2/Bax抑制细胞凋亡,降低TNF-α表达,提高MK、EGFR表达而促进溃疡愈合,提高溃疡愈合质量.AIM:To elucidate the therapeutic mechanism of Jian-Wei-Yu-Yang (JWYY) granules.METHODS:The authors used Okabe’s method to establishgastric ulcer models in rats. Seventy-six SD rats were ran-domly divided into four groups. Except the normal controlgroup, the other three groups were operated and treatedwith distilled water, WeiSu and JWYY respectively. After 7or 14 days of treatment, the ulcer tissues of stomach werecollected to examine ulcer , morphological changes and theother indices. The protein of Midkine (MK), TNF-α,EGFR,Bcl-2 and Bax were detected with immunohistochemistry,in situ hybridization , and cell apoptosis detected with TUNEL.RESULTS:After 7 day and 14 day,the index of ulcer (1.7 ± 0.5,0.8 ± 0.4) and apoptosis (3.0 ± 0.4,2.0 ± 0.5) of JWYYgroup decreased significantly than that of model and WeiSugroup (P <0.01), but the protein and mRNA of MK (33.7± 3.7,38.0 ± 4.1;22.6 ± 2.9,30.5 ± 3.7) and EGFR (39.6 ± 4.9,31.9 ± 6.8;26.6 ± 5.0,17.4 ± 1.6) increased remarkablythan that of the two groups (P <0.05 or 0.01). The TNF-αprotein (18.7±3.3) and mRNA (11.4±3.0) of JWYY groupin 7 day were lower than that in model group (26.1±3.8,17.1±2.7; P <0.01) and higher than that in normal group(4.0±0.9, 3.3±1.0; P <0.01), but they all had no statis-tic difference in 14 day (P >0.05). In both 7 day and 14day, the Bcl-2 protein (33.5 ± 6.8, 32.9 ± 9.6), mRNA(26.1 ± 4.5, 23.0 ± 6.2),protein Bcl-2/Bax (1.3 ± 0.3,1.7±0.7) and mRNA Bcl-2/Bax (1.4±0.3, 1.5±0.4) of JWYYgroup were significantly higher than those of model group(P <0.01 or 0.05).CONCLUSIONS:JWYY granule can cure gastric ulcer and improve quality of ulcer healing by increasing expression ofBcl-2,Bcl-2/Bax, MK and EGFR and decreasing that of TNF-αand cell apoptosis.
