妊娠大于41周时脐动脉及子宫动脉血流动力学的研究

摘　　要：目的　探讨妊娠大于 41周时监测脐动脉和子宫动脉血流动力学指数的临床意义。方法　观察对象为 94例妊娠满 41周至 43周的孕妇 ,用超声多普勒检测仪分别检测其脐动脉和子宫动脉以获得血流动力学指数 :S/D比值、PI、RI和FVR ,按围产儿结局分为正常组和不良组进行对比性研究。结果　①围产儿结局不良组的脐动脉血流各项指数与正常组有显著差异 ,子宫动脉血流的S/D比值和RI有显著差异。②当S/D≥ 2 .5时 ,围产儿结局不良比例与正常比有显著差异。结论　①脐动脉和子宫动脉血流动力学指标能反映出胎儿 胎盘循环和胎盘 子宫循环的状况 ,胎盘退化的程度和对胎儿的影响。②当脐动脉血流指数S/D比值≥ 2 .5时就要考虑胎儿宫内环境不良 ,应及早处理。Objective To study the clinical value of umbilical arteries & uterine arteries hemodynamic index for pregnancy over 41 weeks. Methods Ninety four cases of pregnant women between 41 and 43 weeks were divided into normal group and abnormal group according to perinatal results.The umbilical arteries and uterine arteries were measured with Doppler ultrasound to obtain hemodynamic index:S/D ratio,PI,RI,FVR and to carry on a comparative research.Results ① Each index of umbilical arteries blood flow of abnormal group of perinatal results had obvious difference from those of normal group,the S/D ratio and RI of uterine arteries blood flow also had obvious difference from those of normal group.② When S/D≥2.5,the bad perinatal results had obvious difference from normal results. Conclusion ① Umbilical arteries and uterine arteries hemodynamic index can reflect the condition of fetoplacental and uteroplacental circulation,extent of placental degeneration and affection to fetus.② When umbilical arteries blood flow S/D ratio ≥2.5,we should consider a unfavorable condition inside the uterus and deal with it as soon as possible.
