复方阿苯达唑治疗和控制土源性线虫感染的效果观察

摘　　要：目的　观察复方阿苯达唑不同剂量治疗人体钩、蛔、鞭虫感染的近远期疗效及其控制疫区土源性线虫流行的效果。方法　将受治对象按性别及钩虫卵高低排列、分层 ,随机分为A、B 2组 ,A组顿服 3片 ,B组顿服 2片。粪检用方口圆底盒饱和盐水漂浮法定量检查钩虫卵 ,定性检查蛔、鞭虫卵。结果　治后半月、半年显示 :2组驱钩、蛔、鞭虫的阴转率分别为 69 3 2 %～ 86 43 % ,69 0 9%～ 10 0 %及 2 8 5 7%～ 46 16% ;钩虫卵减少率为 88 44 %～ 97 49% ;驱蛔虫、鞭虫的阴转率 2组差异无显著性 (P >0 0 5 )。驱钩虫及虫卵阴转率和虫卵减少率 3片组均好于 2片组 (P <0 0 5 )。治后 1年、 2年复查结果显示 :钩、鞭虫感染率均维持在较低水平 ,钩虫为 3 73 %～ 14 84% ,鞭虫为 1 5 2 %～3 46%。虫种变化 :治后半月、半年、 1年复查结果均显示美洲钩虫比例增加。钩蚴皮炎发生率由治前的 2 9 2 8%下降到治后 2年的 7 3 2 % ,差异有显著性 (P <0 0 5 )。结论　复方阿苯达唑 3片和 2片组驱钩、蛔、鞭虫的近远期效果均好 ,驱钩虫 3片组优于 2片组。在疫区每年用复方阿苯达唑 3片或 2片普治 1～ 2次 ,连续 2年 ,能将土源性线虫的感染率降低到较低水平。在钩虫轻度感染地区和驱蛔、鞭虫时 ,可用 2片治疗。Objective To assess the short and long-term therapeutic and control effect of different doses of compound albendazole tablets against human hookworm disease,ascariasis and trichuriasis.Methods The subjets were divided randomly into 2 groups.Group A took 3 tablets,and Group B,2 tablets.The stool was examined by saturated salt solution floating method quantitatively (hookworm egg) or qualitatively ( Trichuris trichiura and Ascaris lumbricoides egg). Results The therapeutic effect of both Group A and B,half a month and half a year after treatment showed that the negative reversion rate of hookworm,ascaris and whipworm eggs were 69.32%～86.43%,69.09%～100% and 28.57%～46.16% respectively;and the reduction rate of hookworm egg was 88.44%～97.49%.There were no significant differences in treatment against ascariasis and trichuriasis between the 2 groups ( P>0.05 ),but in treatment of hookworm disease,Group A was better than Group B in both negative reversion rate and egg reduction rate ( P<0.05 ).The control effect showed that 1 and 2 years after treatment,the infection rate of hookworm disease (3.73%～14.84%) and trichuriasis (1.52%～3.46%) maintained in lower level.Among hookworm disease cases,the ratio of Necator americaus to Ancylostoma duodenale increased in half a month,half a year and one year after treatment.In addition,the ground itch caused by hookworm larvae decreased from 29.28% before treatment to 7.32% 2 years after treatment.Conclusion Compound albendazole tablets showed remarkable short- and long-term therapeutic effect against hookworm disease,ascariasis and trichuriasis in both 2 groups.To treat hookworm disease,the effect of Group A was better than that of Group B.In endemic village,mass treatment with 2 or 3 tablets for 2 successive years,both infection rate and intensity of soil-transmitted nemathelminth infections could decrease to lower level. It is recommended that is hypo-endemic areas mass treatment with 2 tablets every year is good enough to achieve better control effect.
