微脂球载体前列腺素E_1治疗肾病综合征疗效观察

摘　　要：目的　探讨微脂球载体前列腺素E1(LipoPGE1)对肾病综合征的治疗作用。方法　 18例肾病综合征患者 ,采用治疗前后自身对照方法进行研究。给予LipoPGE12 0 μg +10 %Glucose 5 0 0mlivdrip .每天 1次 ,连续 2 8天为一疗程。对血压 15 0 / 90mmHg以上者给予Betaloc 75mg、DHCT 2 5mg/d。在开始LipoPGE1治疗前的 1周内 ,停用血管紧张素转换酶抑制剂和钙离子拮抗剂。在用药开始前和结束后的 3天内 ,测定 2 4h尿量、2 4h尿蛋白定量、尿钠、尿肌酐、血肌酐、血尿素氮、内生肌酐清除率 (Ccr) ,同步测定血白蛋白、谷丙转氨酶、胆固醇、甘油三酯等。结果　用药后较用药前尿量增加 (P<0 .0 5 ) ,尿蛋白明显减少 (P <0 .0 1) ,Ccr明显提高 (P <0 .0 1)。结论　LipoPGE12 0 μg +10 %Glucose 5 0 0mlivdrip每天 1次 ,连续 2 8天 ,可以使肾病综合征患者尿量增加 ,尿蛋白含量降低 。Objective To investigate the effectiveness of liposome bearing prostaglandin E 1 (Lipo PGE 1) in the treatment of nephrotic syndrome (NS).Methods Eighteen NS patients were subjected to self control study before and after treatment. Lipo PGE 1 20 μg was given intravenously for 28 days, once daily. Within 3 days before and 3 days after treatment, 24 h urine volume, 24 h urinary protein and urinic natrium, urine creatinine, serum creatinine, BUN, Ccr, blood albumin, serum total cholesterol, triglyceride and so on were measured.Results After treatment, the patient's 24 urine volume and Ccr was obviously increased ( P < 0.05 and P < 0.01 , respectively), but 24 h urinary protein obviously decreased ( P < 0.01 ).Conclusion Lipo PGE 1 can effectively treat nephrotic syndrome.
