呼吸道合胞病毒下呼吸道感染外周血Th细胞亚群的研究

摘　　要：为探讨RSV下呼吸道感染辅助性T淋巴细胞CD4^+及其亚群CD4^+CD45RA^+、CD4^+CD45RO^+的表达及其意义,用单克隆抗体双色免疫荧光PE/FITC双标记,流式细胞仪检测30例RSV下呼吸道感染患儿急性期外用血单个核细胞(PBMCs)辅助性T淋巴细胞 CD4^+及其亚群CD4^+CD45RA^+、CD4^+CD45RO^+的表达;同时检测15例年龄、性别无差异的健康儿为对照。结果显示,RSV下呼吸道感染组患儿外周血辅助性T淋巴细胞CD4^+明显低于对照组(P<0.05);CD4^+CD45RA^+细胞明显下降(P<0.05);CD4^+CD45RO^+细胞虽稍有下降,但与对照组相比无统计学差异(P>0.05);CD4^+CD45RA^+/CD4^+CD45RO^+比值与对照组相比明显降低(P<0.05)。表明婴幼儿RSV下呼吸道感染急性期的免疫功能紊乱主要表现为辅助性T淋巴细胞CD4^+及其亚群CD4^+CD45RA^+/CD4^+CD45RO^+的失衡。In order to explore the expression and its clinical significance of T-helper(Th) lymphocyte CD4 and its subsets in infants with lower respiratory tract infection(LRTI) due to respiratory syncytial virus(RSV), the identification of RSV infection and the expression of CD4 and its subsets CD4 +CD45RA+ CD4+CD45RO + in peripheral blood mononuclear cells of were prospectively tested by the separate ways of double-labelled PE/FITC assay and flow cytometric analysis during the acute stage of infection in 30 infants with RSV-LRTI and also 15 healthy infants with no age and sex difference as normal controls. The results revealed that the T-helper lymphocyte CD4 was found to be significantly decreased as compared with the normal controls(P<0.05) .Besides, the expression of its subset CD4+CD45RA +was also significantly lower than that in healthy infants(P<0.05), but there was no significant difference in the other subset CD4+CD45RO + between RSV-LRTI group and normal group(P> 0.05), although a somewhat reduction of the expression of this subset really took place in the affected infants in comparison with the normal controls.Derived from the data above, the CD4 +CD45RA V CD4 +CD45RO + ratio was actually also apparently lower than that in normal group(P<C0.05) .Therefore, the conclusion could reach such a concept that the disturbance of immune function appearing in LRTI due to RSV should be particularly focused on the imbalance of the expression of Th lymphocyte CD4 and its subsets.
