21例HIV感染者/艾滋病患者HIV载量及CD4细胞数分析

摘　　要：目的 :分析我国HIV感染者 艾滋病患者CD4细胞数和HIV载量的演变过程、相互关系及其与病程发展的关系。方法 :分别用NASBA方法和流式细胞仪绝对计数法检测 2 1名HIV感染者 艾滋病患者的HIV病毒载量和CD4细胞数。对 15名病例随访 4次以上 ,随访时同样检测HIV病毒载量和CD4细胞数 ,并观察临床变化。结果 :在全部 81组数据中 ,CD4细胞数小于 10 0 mm3的 34组数据HIV载量全部大于 10 4 cp ml,而CD4细胞数大于 4 0 0 mm3的 2 7组数据 ,6 3.6 %HIV载量小于 10 4 cp ml(P <0 .0 5 )。HIV载量越高、CD4细胞数越低 ,二者呈明显的负相关关系 (r=0 .974 )。CD4细胞数低于 30 0 mm3的 9例患者中有 5例在随访过程中出现HIV载量的上升和CD4细胞数的下降 ,2例患者死亡。CD4细胞数大于 4 0 0 mm3的 6名患者HIV载量和CD4细胞处于波动状态 ,临床情况较好。结论 :HIV病毒载量较低和CD4细胞数较高 ,患者的预后较好 。Objective:To analyze the evolution of viral load and CD4 cell count and their interrelation in HIV infected individuals and AIDS patients.Methods:The HIV viral load and CD4 count were measured using NASBA and FACS methods respectively. Twenty one HIV infected individuals and AIDS patients were interviewed for clinical syndromes and tested for viral load and CD4 count. Fifteen of them were followed up more than 4 times. Results:In the 34 examinations with CD4 count less than 100/mm 3, viral load was greater than 10 4cp/ml.Of the 27 examinations with CD4 count greater than 400/mm 3,63.6% showed viral load less than 10 4cp/ml( P <0.05).It was obvious that within CD4 count range of 100-400/mm 3,the greater the viral load, the less the CD4 count. Of the 7 patients with CD4 less than 100/mm 3, 2 died and the other 5 showed increase in viral load and decrease in CD4 count during follow_up examinations.Of the 6 patients with CD4 count greater than 400/mm 3,4 were stable in viral load,CD4 count and clinical syndromes.Conclusions:The viral load and CD4 count were negativety correlated, both of them could be used for the prediction of clinical outcome.
