摘 要:目的 探讨新生儿缺氧缺血性脑病 (HIE)血浆和脑脊液 (CSF)中的一氧化氮 (NO)和谷胱甘肽过氧化物酶(GSH PX)含量变化及其HIE不同时期和不同程度间的相关关系。方法 对HIE患儿第 3天的CSF和HIE患儿的初入院、第 1天、第 3天、恢复期的血浆中的NO和GSH PX进行检测 ,并与正常对照组比较 ;分析NO和GSH PX在它们当中变化原因和意义。结果 HIE患儿第 1天的NO含量最高 ,而GSH PX最低 ,NO和GSH PX分别在初入院、第 1天、第3天与正常对照组中的含量对比有极显著性差异 (P <0 .0 1 ) ;恢复期中NO和GSH PX含量与正常对照组对比无显著性差异 (P >0 .0 5) ;中、重度HIE患儿中第 1天后血浆中NO明显比初入院时、第 3天、恢复期增高 ,而GSH PX却明显降低 ;恢复期NO水平与第 1天中NO水平对比在轻度HIE中无显著性差异 ,而在中、重度HIE中对比均有显著性差异 ;HIE血浆中和脑脊液中NO和GSH PX浓度都呈负相关 ;病情越重NO浓度越高 ,而GSH PX浓度越低。结论 HIE患儿在第 1天中NO升至最高 ,中、重度患儿升高尤为明显 ,恢复期下降至正常水平 ,而GSH PX却相反 ,它们呈负相关 ,说明GSH PX对NO浓度升高有抑制作用 ;推测检测血浆和CSF中NO含量和GSH PX有助于判断HIE患儿病变程度 。Objective To study the expression of the Nitric oxide(NO) and Glutathion Peroxidase(GSH PX) in plasma and cerebrospinal fluid (CSF) of neonate hypoxic ischemic enceph alopathy(HIE) and the relations among their expressions in different periods and degrees of HIE. Methods To measure the concentrations of NO and GSH PX in CSF of newborn infants suffered HIE 3 days after onset and in plasma of newborn infants suffered HIE immediate, the firstday,the third day after admisson and convalescent period and compared with them of normal contrast group. Analysed the reasons and significance of NO and GSH PX changes.Results The NO concentration is the highest in plasma of newborn infants suffered HIE 1 day. The more serious the disease is,the higher the NO concentration is.There is significant difference between the GSH PX concentrations in plasma of HIE and normal contrast group during acute periods,but there no difference in canvalescert period. Conclusion The highest concentration in HIE infant appeared in the firstday, especially in maderacte and sever patient, and falled down to normal in convalescent period.On the contrast, GSH PX shown a negetive correlation suggesting that GSH PX had a conpress effect to NO concontration increation.Testing the concentration of NO and GSH PX in plasma and in CSF can also help to judge the degree of disease and the course of disease.
