心电图机检测下T波峰末间期、校正后的T波峰末间期联合血清心肌钙蛋白、肌酸激酶同工酶水平对急性心肌梗死并发心律失常的诊断价值

摘　　要：目的:研究心电图机检测下T波峰末间期(Tp-Te)、校正后的T波峰末间期(Tp-Tec)联合血清心肌钙蛋白、肌酸激酶同工酶水平对急性心肌梗死并发心律失常的诊断价值。方法:选取2022年1月~2023年12月本院收治的确诊急性心肌梗死、疑似心律失常患者179例作为观察对象,所有患者均联合检测心电图机指标(Tp-Te、Tp-Tec)和心肌损伤物质(心肌钙蛋白、肌酸激酶同工酶),对比三组检测的准确度、灵敏度、特异度、阳性预测值、阴性预测值。结果:心电图机检测的准确度、灵敏度、特异度、阳性预测值、阴性预测值分别为82.13%、86.59%、33.33%、93.42%、18.52%,Kappa=0.146,心肌损伤物质检测的准确度、灵敏度、特异度、阳性预测值、阴性预测值分别为78.21%、81.10%、46.67%、94.33%、18.42%,Kappa=0.164,联合检测的准确度、灵敏度、特异度、阳性预测值、阴性预测值分别为95.53%、96.96%、80.00%、98.15%、70.59%,Kappa=0.726;而金标准结果显示164例确诊急性心肌梗死并发心律失常,心电图机检测联合心肌损伤物质检测对急性心肌梗死并发心律失常的特异度为80.00%,Kappa=0.726,与结果诊断有较高的一致性。结论:心电图机检测联合心肌损伤物质检测的特异度均较高,与结果诊断有较高的一致性。Objective:To study the diagnostic value of Tp-Te,Tp-Tec combined with serum cardiac troponin and creatine kinase isoenzymes in acute myocardial infarction complicated with arrhythmia.Methods:179 patients with acute myocardial infarction and suspected arrhythmia admitted to our hospital from January 2022 to December 2023 were selected.All patients were jointly tested for ECG indicators(Tp-Te,Tp-Tec)and myocardial damage substances(cardiac troponin,cTn);Creatine kinase isoenzymes(CK-MB)were used to compare the accuracy,sensitivity,specificity,positive predictive value and negative predictive value of the three groups.Results:The accuracy,sensitivity,specificity,positive predictive value and negative predictive value of ECG were 82.13%,86.59%,33.33%,93.42%and 18.52%,respectively,and Kappa=0.146.The accuracy,sensitivity,specificity,positive predictive value and negative predictive value of myocardial injury substance detection were 78.21%,81.10%,46.67%,94.33%and 18.42%respectively,Kappa=0.164,and the accuracy,sensitivity,specificity,positive predictive value and negative predictive value of joint detection were 95.53%,96.96%,80.00%,98.15%and 70.59%,respectively,and Kappa=0.726;The gold standard results showed that 164 cases of acute myocardial infarction complicated with arrhythmia were diagnosed,and the specificity of ECG combined with myocardial damage substance detection for acute myocardial infarction complicated with arrhythmia was 80.00%,Kappa=0.726,which was highly consistent with the diagnosis results.Conclusion:The specificity of ECG combined with myocardial injury substance detection is high,which is consistent with the diagnosis results.
