摘 要:目的:比较尿液分析仪与显微镜红细胞计数检验在尿液潜血诊断中的临床价值。方法:选取2023年1月~12月在本院行尿潜血检查的98例患者,采集其中段尿液标本,分别采用下列方法进行检验:(1)尿液分析仪检验;(2)显微镜红细胞计数检验。将尿镜检法的检验结果当作本研究的金标准,评定并比较两种检验方法的阳性检出率及单独、联合诊断效能。结果:在98例患者中,采用尿镜检法进行检验,检出阳性74例(75.51%)。尿液分析仪检验共检出阳性例数59例,阳性检出率为60.20%(59/98);显微镜红细胞计数检验检出阳性例数66例,阳性检出率是67.35%(66/98)。尿液分析仪检验法的阳性检出率低于尿镜检法(P<0.05),而单纯显微镜红细胞计数检验法、尿液分析仪检验法联合显微镜红细胞计数检验法的阳性检出率较尿镜检法,差异均不明显(P>0.05)。与尿液分析仪(67.57%、66.33%)相比,显微镜红细胞计数检验法有着更高的灵敏度(82.43%)、准确度(81.63%)(P<0.05),而联合检验有着更高的灵敏度(93.24%)、特异度(91.67%)与准确度(92.86%)(P<0.05);较之显微镜红细胞计数检验法,联合检验的灵敏度、准确度更高(P<0.05)。结论:在尿液潜血诊断中,显微镜红细胞计数检验法的整体效果较尿液分析仪优,若二者相配合,可进一步提高诊断效能。Objective:To compare the clinical value of urine analyzer and microscopic red blood cell count test in the diagnosis of occult blood in urine.Methods:98 patients who underwent urine occult blood tests in our hospital from January to December 2023 were selected.Middle segment urine samples were collected and tested using the following methods:①Urine analyzer test;②Microscopic red blood cell count test.Using the results of uroscopy as the gold standard for this study,evaluate and compare the positive detection rates and diagnostic efficacy of the two testing methods separately and in combination.Results:Among the 98 patients,74 cases(75.51%)were detected as positive using uroscopy.The urine analyzer detected a total of 59 positive cases,with a positive detection rate of 60.20%(59/98);Microscopic red blood cell count test detected 66 positive cases,with a positive detection rate of 67.35%(66/98).The positive detection rate of the urine analyzer test method was lower than that of the urine microscopy test method(P<0.05),while the positive detection rates of the simple microscope red blood cell count test method and the urine analyzer test method combined with the microscope red blood cell count test method are not significantly different from those of the urine microscopy test method(P>0.05).Compared with the urine analyzer(67.57%,66.33%),the microscopic red blood cell count test has higher sensitivity(82.43%)and accuracy(81.63%)(P<0.05),while the combined test has higher sensitivity(93.24%),specificity(91.67%),and accuracy(92.86%)(P<0.05);Compared to the microscopic red blood cell count test,the combined test has higher sensitivity and accuracy(P<0.05).Conclusion:In the diagnosis of occult blood in urine,the overall effect of microscopic red blood cell count test is better than that of urine analyzer.If the two are combined,the diagnostic efficiency can be further improved.
