微创经皮肾镜钬激光碎石术联合输尿管软镜碎石术在复杂性肾结石患者一期治疗中的应用观察

摘　　要：目的:探究微创经皮肾镜钬激光碎石术(mPCNL)联合输尿管软镜碎石术(FURL)在复杂性肾结石患者一期治疗中的临床疗效。方法:选取2019年11月~2022年10月本院收取的100例复杂性肾结石一期患者,将其随机分成观察组(采用mPCNL+FURL,50例)和对照组(采用FURL,50例),比较两组结石清除率、围术期指标、并发症、应激反应指标和肾功能。结果:治疗前,两组围手术期指标、超氧化物歧化酶、皮质醇、丙二醛、半胱氨酸蛋白酶抑制剂C、尿素氮、血肌酐水平对比,差异不显著(P>0.05);治疗后,观察组结石清除率、超氧化物歧化酶、皮质醇、丙二醛水平更高,围手术期指标、并发症总发生率、尿素氮、血肌酐、半胱氨酸蛋白酶抑制剂C水平均更少或更低,差异显著(P<0.05)。结论:mPCNL术联合FURL术在复杂性肾结石患者一期治疗中结石清除率高,能够改善肾功能。Objective:To investigate the clinical efficacy of mPCNL combined with FURL in the first stage treatment of CKS patients.Methods:100 CKS stage I patients admitted to our hospital from November 2019 to October 2022 were randomly divided into an observation group(mPCNL+FURL,50 cases)and a control group(FURL,50 cases).Compare the stone clearance rate,perioperative indicators,complications,stress response indicators,and renal function between two groups.Results:Before treatment,there was no significant difference(P>0.05)in perioperative indicators,SOD,Cor,MDA,Cys-C,BUN,Scr levels between the two groups;After treatment,the stone clearance rate,SOD,Cor,MDA levels were higher in the observation group,while perioperative indicators,total incidence of complications,BUN,Scr,Cys-C levels were lower or lower,with significant differences(P<0.05).Conclusion:The combination of mPCNL and FURL surgery has a high stone clearance rate in the first stage treatment of CKS patients and can improve renal function.
