全髋关节置换术与空心钉内固定术治疗移位型股骨颈骨折患者的效果对比

摘　　要：目的:对比人工全髋关节置换术与空心钉内固定术治疗移位型股骨颈骨折的临床效果。方法:回顾性收集2020年12月~2023年12月本院收治的105例移位型股骨颈骨折患者,依据不同手术方法分为观察组(n=53)、对照组(n=52),观察组行全髋关节置换术,对照组行空心钉内固定术。比较两组手术指标、手术前后髋关节功能、炎症因子(白细胞介素-6、红细胞沉降率、C反应蛋白)及并发症发生情况。结果:与对照组相比,观察组出血量、医疗费用较多,住院时间、首次下床时间较短(P<0.05);术后1个月,与对照组相比,观察组髋关节评分较高(P<0.05);与对照组相比,术后3d,观察组血清白细胞介素-6、红细胞沉降率、C反应蛋白水平较低(P<0.05);两组并发症比较,差异无统计学意义(P>0.05)。结论:与空心钉内固定术治疗相比,移位型股骨颈骨折患者行全髋关节置换术治疗能促进髋关节功能快速恢复,减轻人体炎症反应,缩短住院时间和首次下床活动时间,安全性较好。Objective:To compare the clinical effects of total hip replacement(THR)and hollow nail fixation in the treatment of displaced femoral neck fracture(DFNF).Methods:105 patients with DFNF admitted to our hospital during December 2020 to December 2023 were retrospectively collected and divided into study group(n=53)and control group(n=52)according to different surgical methods.The former received THR and the latter received internal fixation with empty heart nail.Surgical indicators,hip function before and after surgery,inflammatory factors[interleukin-6(IL-6),erythrocyte sedimentation rate(ESR),C-reactive protein(CRP)]and complications were compared between the two groups.Results:Compared with the control group,the blood loss and medical expenses of the study group were higher,the hospitalization time and the first time of getting out of bed were shorter(P<0.05).One month after surgery,the hip score of the study group was higher than that of the control group(P<0.05).Compared with the control group,the serum levels of IL-6,ESR and CRP in the study group were lower 3 days after operation(P<0.05).There was no significant difference in complications between the two groups(P>0.05).Conclusion:Compared with hollow nail fixation,THR treatment in DFNF patients can promote the rapid recovery of hip function,reduce inflammation,shorten hospital stay and the first time of getting out of bed,with better safety.
