微创锁定钢板内固定术与切开复位内固定术治疗肱骨近端骨折的效果对比

摘　　要：目的:比较微创锁定钢板内固定术与切开复位内固定术治疗肱骨近端骨折患者的效果。方法:采用随机抽样方法选取2020年12月—2023年1月于苏州市第九人民医院治疗的66例肱骨近端骨折患者作为研究对象,随机分为常规组与研究组,各33例。常规组实施切开复位内固定术,研究组实施微创锁定钢板内固定术,比较两组临床疗效、并发症、手术相关指标、肩关节功能及术后疼痛程度。结果:研究组优良率高于常规组,并发症发生率低于常规组,术中出血量、引流量少于常规组,住院时间、手术用时、切口长度、骨折愈合时间短于常规组,术后肩关节疼痛、日常活动、活动范围、力量评分及总分高于常规组,术后1 d、3 d、7 d视觉模拟评分法(VAS)评分低于常规组,差异有统计学意义(P<0.05)。结论:相较于切开复位内固定术,微创锁定钢板内固定术治疗肱骨近端骨折患者效果显著,减少并发症,改善手术相关指标与肩关节功能,减轻术后疼痛。Objective:To compare the efficacy of effects of minimally invasive locking plate internal fixation and open reduction and internal fixation in the treatment of proximal humeral fractures.Method:A total of 66 patients with proximal humeral fractures who treated in Suzhou Ninth People's Hospital from December 2020 to January 2023 were selected as the study objects by random sampling method,and they were randomly divided into the conventional group and the study group,with 33 cases in each group.The conventional group was given open reduction and internal fixation,and the study group was given minimally invasive locking plate internal fixation,the clinical efficacy,complications,operation-related indicators,shoulder joint function and postoperative pain were compared between two groups.Result:The excellent and good rate of the study group was higher than that of the conventional group,the incidence of complications was lower than that of the conventional group,the intraoperative blood loss and drainage volume were less than those of the conventional group,the hospitalization time,operation time,incision length and fracture healing time were shorter than those of the conventional group,the postoperative shoulder pain,daily activities,range of activities,strength scores and total score were higher than those of the conventional group,and the visual analogue scale(VAS)scores at 1 d,3 d and 7 d after operation were lower than those of the conventional group,the differences were statistically significant(P<0.05).Conclusion:Compared with open reduction and internal fixation,minimally invasive locking plate internal fixation has a significant effect in the treatment of patients with proximal humeral fractures,reduce complications,improve surgery-related indicators and shoulder joint function,and reduce postoperative pain.
