摘　　要：目的:探讨输尿管软镜下钬激光碎石术治疗输尿管上段结石患者的效果。方法:选取2023年1—12月新泰市人民医院收治的86例输尿管上段结石患者作为研究对象,按双色球随机法将患者分为研究组与对照组,各43例。对照组给予微创经皮肾镜术治疗,观察组给予输尿管软镜下钬激光碎石术治疗,比较两组应激指标、血气指标、手术指标、结石清除率及并发症。结果:术后3 d,两组皮质醇(Cor)、白细胞介素-6(IL-6)、C反应蛋白(CRP)较术前提高,但观察组低于对照组,差异有统计学意义(P<0.05);两组术前、术后3 d血气指标比较,差异无统计学意义(P>0.05);观察组术中出血量少于对照组,手术时间和碎石时间短于对照组,差异有统计学意义(P<0.05);两组术后1个月结石清除率比较,差异无统计学意义(P>0.05);观察组术后3个月结石清除率高于对照组,并发症发生率低于对照组,差异有统计学意义(P<0.05);术后1周,两组心理功能、躯体功能、社会功能、物质生活评分较术前提高,且观察组高于对照组,差异有统计学意义(P<0.05)。结论:输尿管软镜下钬激光碎石术治疗输尿管上段结石患者效果显著,能缩短治疗时间,提高结石清除率,并发症发生率更低,术后恢复更快,患者生活质量更高。Objective:To investigate the effect of holmium laser lithotripsy under flexible ureteroscopy in the treatment of patients with upper ureteral calculi.Method:A total of 86 patients with upper ureteral calculi admitted to People's Hospital of Xintai City from January to December 2023 were selected as the research subjects,and they were randomly divided into the observation group and the control group according to the method of double-color ball randomization,with 43 cases in each group.The control group was treated with minimally invasive percutaneous nephrolithotomy,while the observation group was treated with holmium laser lithotripsy under flexible ureteroscopy.The stress indexes,blood gas indexes,surgical indexes,stone clearance rate and complications were compared between two groups.Result:3 d after operation,the levels of cortisol (Cor),interleukin-6 (IL-6) and C-reactive protein (CRP) in two groups were higher than those before operation,but the observation group was lower than the control group,and the differences were statistically significant (P<0.05).There were no significant differences in the blood gas indexes between two groups before and 3 d after operation (P>0.05).The intraoperative blood loss in the observation group was less than that in the control group,and the operation time and lithotripsy time were shorter than those in the control group,and the differences were statistically significant (P<0.05).There was no significant difference in the stone clearance rate between two groups 1 month after operation (P>0.05);The stone clearance rate of the observation group 3 months after operation was higher than that of the control group,and the incidence of complications was lower than that of the control group,and the differences were statistically significant (P<0.05).1 week after operation,the scores of psychological function,physical function,social function and material life in the two groups were higher than those before operation,and those in the observation group were higher than the control gro
