白内障超声乳化吸出联合房角分离术治疗闭角型青光眼联合白内障患者的临床效果

摘　　要：目的:研究白内障超声乳化吸出(Phaco)联合房角分离术(GSL)治疗原发性闭角型青光眼(PACG)合并白内障患者的临床效果。方法:选取2021年5月—2023年12月济南明水眼科医院收治的80例白内障合并PACG患者作为研究对象,随机分为对照组和观察组,每组各40例。对照组采用Phaco联合小梁切除术(Trab)治疗,观察组采用Phaco联合GSL治疗,比较两组治疗效果、中央前房深度(ACD)、角膜内皮细胞密度(ECD)、房角状态与并发症发生情况。结果:术后1个月,两组最佳矫正视力(BCVA)、眼球内压(IOP)、周边虹膜前粘连(PAS)均降低,且观察组低于对照组,差异有统计学意义(P<0.05);术后一个月,两组ACD较治疗前升高,ECD较治疗前降低,观察组ACD与ECD高于对照组,差异有统计学意义(P<0.05);术后1个月,观察组宽房角比例高于对照组,差异有统计学意义(P<0.05);对照组并发症发生率高于观察组,差异有统计学意义(P<0.05)。结论:Phaco联合GSL治疗PACG合并白内障患者手术效果更好,能有效改善患者BCVA、IOP、PAS、ACD与房角状态,且对角膜损伤较小,并发症风险较低。Objective:To observe the clinical outcome of cataract ultrasonic emulsification aspiration (Phaco) combined with atrial angle segmentation (GSL) in the treatment of patients with primary angle-closure glaucoma (PACG) combined with cataract.Method:One hundred patients with primary angle-closure glaucoma (PACG) and cataract admitted to Ji'nan Mingshui Hospital Department of Ophthalmology from May 2021 to December 2023 were selected as the study objects and randomly divided into a control group and an observation group,with 40 cases in each group.The control group was treated with Phaco combined with trabeculectomy (Trab),while the observation group was treated with Phaco combined with GSL.Therapeutic effects,central anterior chamber depth (ACD),corneal endothelial cell density (ECD),chamber angle status,and the incidence of complications were compared between the two groups.Result:One month after surgery,the best corrected visual acuity (BCVA),intraocular pressure (IOP),and peripheral pre-iris adhesion (PAS) were reduced in both groups,and the observation group was lower than the control group,and the difference was statistically significant (P<0.05).One month after surgery,ACD was higher and ECD was lower in both groups than before treatment,and ACD and ECD in the observation group than those in the control group,and the difference was statistically significant (P<0.05).One month after surgery,the proportion of wide atrial angle in the observation group was higher than that in the control group,and the difference was statistically significant (P<0.05).The complication rate of the control group was higher than that of the observation group,and the difference was statistically significant (P<0.05).Conclusion:Phaco combined with GSL can effectively improve BCVA,IOP,PAS,ACD and Angle status of patients with PACG and cataract,and has less damage to cornea and lower risk of complications.
