埋线疗法对老年脑卒中后下肢肌痉挛病人的疗效观察

摘　　要：目的观察埋线疗法对老年脑卒中后下肢肌痉挛病人的临床疗效。方法选取南京医科大学附属老年医院住院和门诊治疗的老年脑卒中后下肢肌痉挛病人40例作为研究对象,采用随机数表法分为治疗组和对照组,每组20例。对照组予常规康复训练,治疗组予埋线治疗联合常规康复训练,比较2组治疗前后的改良Ashworth量表(modified Ashworth scale,MAS)评级、Peen评分、下肢Fugl-Meyer运动功能(FMA)评分和杨氏模量绝对值。结果治疗前,2组MAS评级、Peen评分、下肢FMA评分和杨氏模量绝对值差异均无统计学意义(P>0.05)。治疗后24 h,治疗组MAS评级、Peen评分均优于对照组;治疗后第10天,治疗组MAS评分、Peen评分、下肢FMA评分和杨氏模量绝对值亦优于对照组,差异均有统计学意义(P<0.05)。结论埋线疗法联合常规康复训练能显著提升老年脑卒中后下肢肌痉挛的治疗效果。Objective To observe the clinical efficacy of acupoint thread embedding therapy in the elderly patients with lower limb muscle spasms after stroke.Methods Forty elderly patients with post-stroke spasticity of lower limbs from Affiliated Geriatric Hospital of Nanjing Medical University were randomly divided into the treatment group and the control group,with 20 cases in each group.The control group received routine rehabilitation training,while the treatment group received thread embedding therapy(polydioxanone threads,PDO)combined with routine rehabilitation training.The grade of modified Ashworth scale,Peen scale and the scores of lower limb Fugl-Meyer assessment scale(FMA),and absolute value of Young’s modulus before and after treatment were compared between the two groups.Results There were no statistically significant differences in MAS grade,Peen grade,lower limb FMA score,and absolute value of Young’s modulus between the two groups before treatment(P>0.05).After 24 hours of treatment,MAS grade and Peen grade in the treatment group were more significantly improved than those in the control group;On the 10th day after treatment,MAS score,Peen score,lower limb FMA score,and absolute value of Young’s modulus in the treatment group were more significantly improved than those in the control group(P<0.05).Conclusions Acupoint thread embedding therapy combined with conventional rehabilitation training can significantly improve the therapeutic effect of lower limb muscle spasticity after stroke in the elderly patients.
