尿促性素联合来曲唑治疗克罗米芬抵抗PCOS不孕患者的临床效果分析

摘　　要：目的 探讨尿促性素(HMG)联合来曲唑治疗克罗米芬抵抗多囊卵巢综合征(PCOS)不孕患者的临床效果。方法 选择2021年6月—2023年6月济宁市任城区妇幼保健院收治的80例克罗米芬抵抗PCOS不孕患者为研究对象,按随机数字表法将其分为对照组和观察组,各40例。对照组采用来曲唑治疗,观察组在对照组基础上加用HMG治疗。对比两组的辅助生殖指标、雌激素水平、妊娠、排卵及流产情况、不良反应发生情况。结果 治疗后,观察组人绒毛膜促性腺激素注射日≥18 mm卵泡数多于对照组,子宫内膜厚度厚于对照组,卵泡成熟时间短于对照组,宫腔容积大于对照组,组间差异有统计学意义(P<0.05);观察组卵泡刺激素高于对照组,雌二醇、促黄体生成素均低于对照组,组间差异有统计学意义(P<0.05);观察组妊娠率为52.50%,排卵率为67.50%,均高于对照组的30.00%、45.00%,组间差异有统计学意义(P<0.05);两组流产率、不良反应发生率对比,组间差异无统计学意义(P>0.05)。结论 在克罗米芬抵抗PCOS不孕患者中应用尿促性素联合来曲唑治疗可有效提升辅助生殖指标,改善雌激素水平,提高排卵率及妊娠率,且用药安全。Objective To investigate the clinical effect of human menopausal gonadotropins(HMG) and letrozole in the treatment of clomiphene resistance in polycystic ovary syndrome(PCOS) infertility patients.Methods Eighty clomiphene resistance PCOS infertility patients admitted to Rencheng District Maternal and Child Health Hospital in Jining City from June2021 to June 2023 were selected as the research objects,and were divided into a control group and an observation group according to random number table method,with 40 cases in each group.The control group was treated with letrozole and the observation group was treated with HMG on the basis of the control group.The assisted reproductive indicators,estrogen,pregnancy,ovulation and abortion,and the occurrence of adverse reactions were compared between the two groups.Results After treatment,the number of follicles ≥18 mm on human chorionic gonadotropin injection day in the observation group was higher than that in the control group,the endometrial thickness was thicker than that in control group,the maturation time of follicles was shorter than that in the control group,and the uterine volume was larger than that in the control group,and the differences between the groups were statistical significance(P<0.05);the follicle-stimulating hormone in observation group was higher than that in the control group,estradiol and luteinizing hormone were lower than those in the control group,and the differences between the groups were statistically significant(P <0.05);the pregnancy rate and ovulation rate in the observation group were 52.50% and 67.50%,respectively,which were higher than 30.00% and 45.00% in the control group,and the differences between the groups were statistical significance(P<0.05);there was no significant difference in the rate of abortion and incidence of adverse reactions between the two groups(P>0.05).Conclusion In clomiphene resistance to PCOS infertility patients,the treatment of human menopausal gonadotropins combined with letrozole can effectively improve t
