摘 要:目的:分析自身免疫性甲状腺炎(AIT)患者血清白细胞介素-10(IL-10)、白细胞介素-17(IL-17)、白细胞介素-35(IL-35)、γ干扰素(INF-γ)水平及临床意义。方法:选取2021年1月—2023年12月惠安县医院78例AIT患者纳入AIT组,并取同期40例体检健康者纳入对照组,比较两组甲状腺检测指标[游离甲状腺素(FT_(4))、游离三碘甲腺原氨酸(FT_(3))、甲状腺球蛋白抗体(TGAb)及甲状腺过氧化物酶抗体(TPOAb)]、血清IL-10、IL-17、IL-35、INF-γ水平,使用Pearson法分析甲状腺检测指标与IL-10、IL-17、IL-35、INF-γ相关性,使用logistic回归模型分析AIT的影响因素。结果:两组FT_(4)、FT_(3)比较,差异无统计学意义(P>0.05);AIT组TGAb、TPOAb水平高于对照组,差异有统计学意义(P<0.05);AIT组血清IL-17、INF-γ水平高于对照组,IL-10、IL-35水平低于对照组,差异有统计学意义(P<0.05);IL-10、IL-35与TGAb、TPOAb呈负相关,IL-17、INF-γ与TGAb、TPOAb呈正相关(P<0.05);IL-10、IL-35为AIT的保护因素,IL-17、INF-γ为AIT的危险因素(P<0.05)。结论:AIT患者存在血清IL-10、IL-35水平异常降低及IL-17、INF-γ水平异常升高变化,且与AIT病情密切相关,IL-10、IL-35为AIT的保护因素,IL-17、INF-γ为AIT的危险因素。Objective:To analyze the levels and clinical significance of serum interleukin-10(IL-10),interleukin-17(IL-17),interleukin-35(IL-35)and interferon-γ(INF-γ)in patients with autoimmune thyroiditis(AIT).Method:A total of 78 patients with AIT admitted to Hui’an County Hospital from January 2021 to February 2023 as the AIT group,and 40 healthy individuals who received health check-ups in the hospital were selected at the same time as the control group.Thyroid detection indicators[free thyroxine(FT_(4)),free triiodothyronine(FT_(3)),thyroglobulin antibody(TGAb)and thyroid peroxidase antibody(TPOAb)]and serum levels of IL-10,IL-17,IL-35 and INF-γwere compared between the two groups.Pearson method was used to analyze the correlation between thyroid detection indicators and IL-10,IL-17,IL-35 and INF-γ.logistic regression model was used to analyze the influencing factors of AIT.Result:There were no statistically significant differences in FT_(4),FT_(3) between the two groups(P>0.05).The levels of TGAb and TPOAb in AIT group were higher than those in control group,the differences were statistically significant(P<0.05).Serum IL-17 and INF-γlevels in AIT group were higher than those in control group,the levels of IL-10 and IL-35 were lower than those in control group,the differences were statistically significant(P<0.05).IL-10 and IL-35 were negatively correlated with TGAb and TPOAb,IL-17 and INF-γwere positively correlated with TGAb and TPOAb(P<0.05).IL-10 and IL-35 were protective factors for AIT,IL-17 and INF-γwere risk factors for AIT(P<0.05).Conclusion:There are abnormal decreases in serum IL-10 and IL-35 levels,and abnormally increases in the levels of IL-17 and INF-γin patients with AIT.Above indicators are closely related to the condition of AIT.IL-10 and IL-35 are protective factors for AIT,while IL-17 and INF-γare risk factors.
