摘  要:目的观察电针对糖尿病胃轻瘫(DGP)大鼠肠道菌群特定菌属含量和肠黏膜相关免疫因子的影响。方法SD大鼠随机分为空白对照组、DGP模型组、甲氧氯普胺(胃复安)组、电针组。采用一次性腹腔注射链脲佐菌素(STZ)结合高脂高糖不规律饮食制备DGP模型。胃复安组以1 ml/100 g的剂量予1.7%浓度的胃复安药液灌胃;电针组取同侧“足三里”“梁门”“三阴交”电针治疗。血糖仪测量血糖、酚红灌胃测量胃排空率及小肠推进率,实时荧光定量聚合酶链反应(PCR)技术测定致病菌肠球菌,益生菌乳酸杆菌、双歧杆菌、拟杆菌含量,酶联免疫吸附试验(ELISA)检测血清内毒素、血清免疫球蛋白(IgA)、回肠分泌型免疫球蛋白(sIgA)含量、肠道炎症因子白细胞介素(IL)-6、IL-8、IL-10、肿瘤坏死因子(TNF)-α水平,免疫组化法检测肠黏膜T淋巴细胞亚群CD3^(+)、CD4^(+)、CD8^(+)表达,苏木素-伊红(HE)染色法观察肠黏膜结构。结果造模前,各组血糖值无明显差异(P>0.05)。造模后,与空白对照组相比,DGP模型组血糖值显著上升,胃排空率与小肠推进率显著降低,致病菌数量显著增多,有益菌数量显著减少,血清内毒素含量明显增加,IgA水平明显减少,炎症因子IL-6、IL-8、TNF-α水平明显增多,抗炎因子IL-10含量明显减少,CD3^(+)、CD4^(+)表达明显降低,CD8^(+)表达明显升高(P<0.05,P<0.01),肠黏膜结构损害严重;干预后,与DGP模型组相比,电针组和胃复安组血糖值显著下降,胃排空率与小肠推进率显著升高,致病菌明显减少,有益菌显著增多,血清内毒素含量明显减少,IgA和sIgA水平明显增高,致炎因子IL-6、IL-8、TNF-α水平明显减少,抗炎因子IL-10水平明显增多,CD3^(+)、CD4^(+)细胞均显著高表达,CD8^(+)显著低表达(P<0.05,P<0.01),肠黏膜结构损害程度较轻;与胃复安组相比,电针组CD3^(+)、IL-8含量显著升高,其余指标差异均无统计学意义(P>0.05)。结论�Objective To observe the effect of electroacupuncture on the content of specific bacteria in intestinal flora and intestinal mucosal related immune factors in diabetic gastroparesis(DGP)rats.Methods SD rats were randomly divided into blank control group,DGP model group,metoclopramide group and electroacupuncture group.DGP model was established by one-time intraperitoneal injection of streptozotocin(STZ)combined with irregular diet with high fat and sugar.Metoclopramide group was given 1.7%metoclopramide solution at the dose of 1 ml/100 g,while the electroacupuncture group took the same side"Zusanli""Liangmen"and"Sanyinjiao"electroacupuncture treatment.The blood glucose meter was used to measure the blood glucose,the gastric emptying rate and intestinal propulsion rate were measured by intragastric administration of phenol red,and the contents of pathogenic bacterium including Enterococcus,probiotic including Lactobacillus,Bifidobacterium and Bacteroides were measured by real-time fluorescence quantitative polymerase chain reaction(PCR).Serum endotoxin,contents of serum immunoglobulin(IgA)and ileal secretory immunoglobulin(sIgA),levels of intestinal inflammatory factors interleukin(IL)-6,IL-8,IL-10 and tumor necrosis factor(TNF)-αwere detected by enzyme linked immunosorbent assay(ELISA),and expressions of T lymphocyte subsets CD3^(+),CD4^(+)and CD8^(+)in intestinal mucosa were detected by immunohistochemistry.The intestinal mucosal structure was observed by hematoxylin-eosin(HE)staining.Results Before modeling,there was no significant difference in blood glucose levels among the groups(P>0.05).After modeling,compared with the blank control group,the blood glucose level of DGP model group was significantly increased,the gastric emptying rate and intestinal propulsion rate were significantly decreased,the number of pathogenic bacteria was significantly increased,the number of beneficial bacteria was significantly decreased,the content of serum endotoxin was significantly increased,and the level of IgA was significa
