活血生肌方联合创面封闭式负压引流治疗下肢静脉曲张伴溃疡临床研究

摘　　要：目的:观察活血生肌方联合创面封闭式负压引流治疗下肢静脉曲张伴溃疡的临床效果。方法:选取2022年1月—2023年10月嘉兴市中医医院收治的88例下肢静脉曲张伴溃疡患者,按随机数字表法分为对照组和观察组各44例。对照组给予0.9%的氯化钠溶液+封闭式负压引流治疗,观察组给予活血生肌方+封闭式负压引流治疗,治疗2周。评价2组临床疗效,比较2组治疗前后溃疡创面症状评分、血管新生指标以及炎性因子水平。结果:治疗后,观察组总有效率为88.64%(39/44),高于对照组70.45%(31/44)(P<0.05)。治疗后,2组溃疡创面各项症状评分较治疗前降低(P<0.05),且观察组溃疡创面各项症状评分低于对照组(P<0.05)。治疗后,2组内皮抑素(ES)水平较治疗前降低,且观察组低于对照组(P<0.05);2组碱性成纤维细胞生长因子(bFGF)、血管内皮细胞生长因子(VEGF)水平较治疗前升高,且观察组高于对照组(P<0.05)。治疗后,2组肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)水平较治疗前降低(P<0.05),且观察组TNF-α、IL-1β、IL-6水平低于对照组(P<0.05)。结论:活血生肌方联合创面封闭式负压引流治疗下肢静脉曲张伴溃疡疗效显著,能够促进创面愈合和创面微血管内皮生长,加速血管生成,减轻机体炎性反应。Objective:To observe the clinical effect of Huoxue Shengji Prescription combined with closed negative pressure wound drainage on varicose veins of lower extremity with ulcer.Methods:A total of 88 patients with varicose veins of lower extremity with ulcer admitted to Jiaxing Hospital of Traditional Chinese Medicine from January 2022 to October 2023 were selected and randomly divided into the control group and the observation group,with 44 patients in each group.The control group was given 0.9%of sodium chloride solution combined with closed negative pressure drainage,and the observation group was given Huoxue Shengji Prescription combined with closed negative pressure drainage.Both groups were treated for two weeks.The clinical effects in the two groups were evaluated;the wound symptom scores,angiogenesis indexes and levels of inflammatory factors before and after treatment were compared between the two groups.Results:After treatment,the total effective rate was 88.64%(39/44)in the observation group,which was higher than that of 70.45%(31/44)in the control group(P<0.05).After treatment,the wound symptom scores in the two groups were decreased when compared with those before treatment(P<0.05),and the wound symptom score in the observation group was lower than that in the control group(P<0.05).After treatment,the levels of endostatin(ES)in the two groups were reduced when compared with those before treatment,and the level in the observation group was lower than that in the control group(P<0.05).The levels of basic fibroblast growth factor(bFGF)and vascular endothelial growth factor(VEGF)in the two groups were elevated when compared with those before treatment,and the levels in the observation group were higher than those in the control group(P<0.05).After treatment,the levels of tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β)and interleukin-6(IL-6)in the two groups were declined when compared with those before treatment(P<0.05),and the levels of TNF-α,IL-1βand IL-6 in the observation group were lower than
