中药定向透药联合头针对老年脑卒中后偏瘫患者患肢周径、血流和肌力的影响

摘　　要：目的:观察中药定向透药联合头针对老年脑卒中后偏瘫患者患肢周径、血流和肌力的影响。方法:选择2022年1月—2023年1月丽水市人民医院住院治疗的198例脑卒中后偏瘫老年患者,按随机数字表法分为治疗组及对照组各99例。对照组接受常规康复治疗,治疗组在对照组基础上增加中药定向透药联合头针治疗。比较2组临床疗效,比较2组治疗前后中医症状积分、患肢周径、颈椎动脉血管阻力指数(RI)、患肢肌力评分的变化。结果:治疗后,2组关节肿胀、关节疼痛、关节活动范围中医症状积分均较治疗前下降(P<0.05),治疗组上述3项中医症状积分均低于对照组(P<0.05)。治疗后,2组患肢周径均较治疗前缩小(P<0.05),治疗组患肢周径小于对照组(P<0.05)。治疗后,2组颈动脉RI、椎动脉RI均较治疗前下降(P<0.05),治疗组颈动脉RI、椎动脉RI均低于对照组(P<0.05)。治疗后,2组患肢肌力评分均较治疗前上升(P<0.05),治疗组患肢肌力评分高于对照组(P<0.05)。治疗组临床疗效总有效率为90.91%(90/99),对照组为71.72%(71/99),2组比较,差异有统计学意义(P<0.05)。结论:中药定向透药联合头针可改善老年脑卒中后偏瘫患者的患肢周径、血流和肌力,对老年脑卒中后偏瘫患者康复治疗起到积极作用。Objective:To observe the effects of Chinese medicine-directed penetration combined with scalp acupuncture on circumference,blood flow,and muscle strength of the affected limbs of senile patients with post-stroke hemiplegia.Methods:A total of 198 senile patients with post-stroke hemiplegia who were hospitalized at Lishui People's Hospital from January 2022 to January 2023 were selected and divided into the treatment group and the control group according to the random number table method,with 99 cases in each group.The control group received conventional rehabilitation treatment,and the treatment group received Chinese medicine-directed penetration combined with scalp acupuncture in addition to the conventional rehabilitation treatment.Clinical effects were compared between the two groups;the changes in traditional Chinese medicine symptom scores,circumference of the affected limbs,cervical artery resistance index(RI),and limb muscle strength scores in the two groups were compared before and after treatment.Results:After treatment,traditional Chinese medicine symptom scores for joint swelling,joint pain,and range of joint motion in the two groups were decreased when compared with those before treatment(P<0.05),and the above three traditional Chinese medicine symptom scores in the treatment group were lower than those in the control group(P<0.05).Circumferences of the affected limbs in the two groups were reduced when compared with those before treatment(P<0.05),and the circumference in the treatment group was smaller than that in the control group(P<0.05).Cervical artery RI and vertebral artery RI in the two groups were reduced when compared with those before treatment(P<0.05),with the treatment group showing lower RI values than the control group(P<0.05).After treatment,muscle strength scores of the affected limbs in the two groups were increased when compared with those before treatment(P<0.05),with the treatment group scoring higher than the control group(P<0.05).The total effective rate of clinical effect was 9
