眼袋整形术联合自体脂肪移植填充在老年性泪沟型眼袋治疗中的应用

摘　　要：目的探讨眼袋整形术联合自体脂肪移植填充在老年性泪沟型眼袋治疗中的应用效果。方法对2021年3月至2023年1月于成都市恩喜医疗美容医院52例老年性眼袋伴泪沟凹陷畸形患者进行回顾性分析。所有患者均行泪沟型眼袋矫正术。手术采用下睑缘皮肤入路,适量去除皮肤、肌肉及眶隔脂肪,大腿内侧抽取脂肪并制备为颗粒脂肪,并用18G注脂针将其移植填充于泪沟和部分眶周区域。结果52例患者术后随访1周、3~6个月,术后眼袋及泪沟凹陷畸形均得到明显改善,且达到眶下区年轻化的效果;1例术后血肿形成,3例术后泪沟畸形复发。结论眼袋整形术同时联合自体脂肪移植填充应用于伴泪沟凹陷畸形的老年眼袋患者,手术操作简便,术后美容效果良好,并发症发生率低,值得推广。Objective To investigate the effect of pouch plastic surgery combined with autologous fat transplantation in the treatment of senile lacrimal pouch.Methods A retrospective analysis was conducted on 52 elderly patients with eye bags accompanied by lacrimal groove depression deformity at Enxi Medical Beauty Hospital in Chengdu from March 2021 to January 2023.All patients underwent lacrimal groove type eye bag correction surgery.The surgery adopts the skin approach of the lower eyelid margin,with appropriate removal of skin,muscle,and orbital fat.Fat is extracted from the inner thigh and prepared into granular fat,which is then transplanted and filled into the lacrimal groove and some periorbital areas using an 18G liposuction needle.Results Fifty-two patients were followed up for 1 week and 3 to 6 months after surgery,and significant improvements were observed in the deformity of eye bags and tear troughs,achieving a rejuvenation effect in the infraorbital region.One case of postoperative hematoma formation and three cases of postoperative lacrimal groove malformation recurrence.Conclusions Pouch plastic surgery combined with autologous fat transplantation for the elderly patients with lacrimal groove depression is simple and convenient,with good cosmetic effect and low complication rate.
