切削痂植皮术联合负压封闭引流技术治疗手部深度烧伤的回顾性分析

摘　　要：目的探讨创面切削痂植皮术联合负压封闭引流术(VSD)治疗手部深度烧伤的效果。方法收集2017年8月至2023年12月联勤保障部队第九八八医院收治的94例手部深度烧伤患者的临床资料进行回顾性研究。按照治疗方案分组,接受创面切削痂植皮术的46例患者为对照组,接受面切削痂植皮术联合VSD的48例患者为研究组。比较两组患者植皮存活状况、创面恢复状况、手指功能状况、并发症状况。结果术后2周,研究组植皮存活率为(92.85±2.63)%,高于对照组(79.68±2.77)%,差异有统计学意义(P<0.05);研究组患者创面愈合时间、肉芽长出时间与血流运行恢复时间短于对照组,差异有统计学意义(P<0.05);术后6个月,研究组患者翻转时间、放置时间短于对照组,患手、双手、组装数多于对照组,差异有统计学意义(P<0.05);术后1周,研究组患者并发症发生率4.17%,低于对照组21.74%,差异有统计学意义(P<0.05)。结论创面切削痂植皮术联合VSD治疗手部深度烧伤效果显著,可提高植皮存活率,改善创面恢复效果。Objective To explore the effect of wound excision scab grafting combined with negative pressure closure drainage(VSD)in the treatment of deep burns of the hand.Methods A retrospective study was conducted on the clinical data of 94 patients with deep burns of the hands admitted to the 988th Hospital of the Joint Logistics Support Force from August 2017 to December 2023.According to the treatment plan,46 patients who underwent wound excision and scab grafting were selected as the control group,and 48 patients who underwent surface excision and scab grafting combined with VSD were selected as the study group.Compare the survival status of skin grafts,wound recovery,finger function,and complications between two groups of patients.Results Two weeks after surgery,the survival rate of skin grafting in the study group was(92.85±2.63)%,which was higher than that in the control group(79.68±2.77)%,and the difference was statistically significant(P<0.05);The wound healing time,granulation tissue growth time,and blood flow recovery time of the study group were shorter than those of the control group,and the difference was statistically significant(P<0.05);Six months after surgery,the flipping time and placement time of the study group patients were shorter than those of the control group,and the number of affected hands,hands,and assemblies was higher than that of the control group,with statistical significance(P<0.05);One week after surgery,the incidence of complications in the study group was 4.17%,which was lower than the control group's 21.74%,and the difference was statistically significant(P<0.05).Conclusions The combination of wound scab excision and skin grafting with VSD has a significant effect on the treatment of deep burns in the hands,which can improve the survival rate of skin grafting and enhance wound recovery.
