摘 要:目的探讨308 nm准分子激光联合火针对白癜风皮损改善情况及抗氧化应激能力的影响。方法选取2022年1月至2023年3月于濮阳市安阳地区医院门诊部就诊的白癜风患者98例,根据随机数字表法将98例白癜风患者分为联合组(49例)和光疗组(49例)。光疗组采用308 nm准分子激光疗法,联合组在光疗组的基础上同时给予火针疗法,两组患者每周治疗2次,共治疗8周。观察两组治疗前后皮损面积、皮损色素积分以及细胞内活性氧(ROS)、超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)水平的变化情况,比较两组治疗8周后的临床疗效,记录两组治疗期间不良反应发生情况。结果联合组治疗总有效率高于光疗组总有效率(P<0.05);治疗前,两组患者皮损面积、皮损色素积分以及ROS、SOD、GSH-Px水平比较,差异无统计学意义(P>0.05),治疗后,两组皮损面积、ROS水平降低(P<0.05),且联合组小于光疗组(P<0.05),两组皮损色素积分、SOD、GSH-Px水平升高,且联合组高于光疗组(P<0.05);治疗期间联合组不良反应总发生率低于光疗组,但差异无统计学意义(P>0.05)。结论308 nm准分子激光联合火针疗法对白癜风患者有理想的治疗效果,能够减少白癜风患者的皮损面积,提升患者皮损部位色素沉着,有效降低ROS水平,提高SOD、GSH-Px水平,改善患者抗氧化应激能力。Objective To discuss the effect of 308 nm excimer laser combined with fire needle therapy on the improvement of vitiligo-like lesions and resistance to oxidative stress.Methods Ninety-eight patients with vitiligo in the outpatient department of Puyang City Anyang District Hospital from January 2022 to March 2023 were enrolled,and randomly assigned into two groups,each with 49 cases.Light therapy group was given 308 nm excimer laser therapy,while combined group received 308 nm excimer light combined with fire needle therapy.Treatment in both groups was twice a week for a total of 8 weeks.Parameters including lesion area,lesion pigmentation score,intracellular reactive oxygen species(ROS),superoxide dismutase(SOD)and glutathione peroxidase(GSH-Px)were observed before and after treatment in both groups.Meantime,the clinical efficacy rate after 8 weeks of treatment and adverse reactions during treatment period were compared between two groups.Results The total efficacy rate in combined group was higher than that in light therapy group(P<0.05).There were no differences in the lesion area and lesion pigmentation score as well as serum levels of ROS,SOD and GSH-Px between two groups before treatment(P>0.05).After treatment,a decrease in lesion area and ROS level was found in both groups,and the decrease was more notable in combined group than those in light therapy group(P<0.05).An increase in lesion pigmentation score and serum levels of SOD and GSH-Px was detected in both groups after treatment,and the increase was more remarkable in combined group than those in light therapy group(P<0.05).The total adverse reaction rate in combined group was lower than that in light therapy group,with no statistical difference(P>0.05).Conclusion Application of 308 nm excimer laser combined with fire needle therapy in patients with vitiligo can effectively produce a rapid decrease in size of the lesion,alleviation of lesion pigmentation,reduction in ROS level,increase in SOD and GSH-Px level,and improvement in resistance to oxidative
