摘  要:目的探究三维重建技术联合CT引导下Hook-wire穿刺定位技术在肺磨玻璃结节(ground-glass nodule,GGN)胸腔镜肺段切除术中的应用效果。方法选取2020年1月至2022年1月于九江市第一人民医院行单孔胸腔镜肺段切除的75例肺GGN患者作为研究对象,按入院先后顺序分为对照组(n=40)与实验组(n=35)。实验组应用三维重建技术联合CT引导下Hook-wire穿刺定位技术,对照组单独应用三维重建技术。比较两组结节距离、定位准确率、手术相关指标及并发症发生情况。结果两组结节与脏层胸膜、斜裂或水平裂的距离比较差异无统计学意义;实验组定位准确率高于对照组,差异有统计学意义(P<0.05)。实验组手术时间、引流管留置时间、术后住院时间均短于对照组,住院费用少于对照组,差异有统计学意义(P<0.05)。实验组并发症发生率为8.57%,低于对照组的27.50%,差异有统计学意义(P<0.05)。结论三维重建技术联合CT引导下Hook-wire穿刺定位技术应用于肺GGN胸腔镜肺段切除术中,可更精准地切除解剖性肺段,手术安全性更高,有效减少并发症的发生,缩短患者住院时间,减少治疗费用,具有良好的社会效益和经济效益。Objective To explore the application effect of 3D reconstruction technology combined with CT-guided Hook-wire puncture localization technology in thoracoscopic segmentectomy of pulmonary ground-glass nodule(GGN).Methods 75 pulmonary GGN patients who underwent single-orifice thoracoscopic segmentectomy in The First People's Hospital of Jiujiang City from January 2020 to January 2022 were selected as the study subjects,and they were divided into the control group(n=40)and experimental group(n=35)according to the order of admission.The experimental group applied 3D reconstruction technology combined with Hook-wire puncture positioning technology guided by CT,and the control group applied 3D reconstruction technology alone.The nodule distance,localization accuracy,surgery-related indexes and the occurrence of adverse events were compared between the two groups.Results There was no significant difference in the distance between nodules and visceral pleura,oblique fissure or horizontal fissure between the two groups.The localization accuracy in the experimental group was higher than that in the control group,and the difference was statistically significant(P<0.05).The operation time,drainage tube retention time and postoperative hospitalization time in the experimental group were shorter than those in the control group,and the hospitalization cost was less than that in the control group,the differences were statistically significant(P<0.05).The incidence of adverse events in the experimental group was 8.57%,which was lower than 27.50%in the control group,the difference was statistically significant(P<0.05).Conclusion The application of 3D reconstruction technology combined with CT-guided Hook-wire puncture localization technology in thoracoscopic segmentectomy with pulmonary GGN can make the resection of anatomical lung segments more accurate,with higher surgical safety,effectively reduce the occurrence of complications,shorten the length of hospital stay,and reduce the cost of treatment,which has good social and econo
