醋酸曲普瑞林辅助治疗特发性中枢性性早熟患儿的临床疗效

摘　　要：目的探讨醋酸曲普瑞林辅助治疗特发性中枢性性早熟(idiopathic central precocious puberty,ICPP)患儿的临床疗效。方法选取2019年8月至2021年8月南平市妇幼保健院收治的102例ICPP患儿作为研究对象,按照随机数字表法分为对照组与观察组,每组51例。对照组采用常规治疗,观察组在对照组基础上加用醋酸曲普瑞林治疗。比较两组血清性激素水平[雌激素(estradiol,E_(2))、促黄体生成素(luteinizing hormone,LH)、卵泡刺激素(follicle-stimulating hormone,FSH)]、卵巢容积及长径、子宫容积及长径、骨龄、临床疗效、不良反应发生情况。结果治疗后,观察组E_(2)、LH、FSH水平均低于对照组,差异有统计学意义(P<0.05)。治疗12个月后,观察组卵巢容积、子宫容积及骨龄均小于对照组,卵巢长径、子宫长径均短于对照组,差异有统计学意义(P<0.05)。观察组治疗总有效率为96.08%,高于对照组的82.35%,差异有统计学意义(P<0.05)。两组治疗期间均未出现不良反应。结论醋酸曲普瑞林辅助治疗ICPP效果显著,能有效调节患儿血清性激素水平,延缓骨龄生长,抑制第二性征发育,且安全性较高,值得临床推广应用。Objective To investigate the clinical efficacy of treprostinil acetate in the adjuvant treatment of children with idiopathic central precocious puberty(ICPP).Methods 102 children with ICPP admitted to Nanping Maternal and Child Health Care Hospital from August 2019 to August 2021 were selected as the research subjects,they were divided into the control group and the observation group according to random number table method,with 51 cases in each group.The control group received conventional treatment,while the observation group was treated with triptorelin acetate on top of the control group.The serum levels of sex hormones(estrogen[E_(2)],luteinizing hormone[LH],follicle-stimulating hormone[FSH]),ovarian volume and length,uterine volume and length,bone age,clinical efficacy and adverse reactions were compared between the two groups.Results After treatment,the levels of E_(2),LH and FSH in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After 12 months of treatment,the ovarian volume,uterine volume and bone age in the observation group were smaller than those in the control group,and the ovarian and uterine diameters were shorter than those in the control group,and the differences were statistically significant(P<0.05).The total effective rate in the observation group was 96.08%,which was higher than 82.35%in the control group,and the difference was statistically significant(P<0.05).No adverse effects were observed during treatment in either group.Conclusion Triptorelin acetate is effective in the adjuvant treatment of ICPP girls,and can effectively regulate serum sex hormone levels,delay bone age growth,inhibit the development of secondary sexual characteristics,and has high safety,which is worthy of clinical promotion and application.
