郭氏正骨手法治疗先天性肌性斜颈的临床有效性

摘　　要：目的分析先天性肌性斜颈运用郭氏正骨手法治疗的临床疗效。方法选取2021年4月至2022年4月莆田市第一医院中医科接诊的60例先天性肌性斜颈患儿,采用随机数字表法分为对照组(30例)和观察组(30例)。对照组应用常规推拿治疗,观察组运用郭氏正骨手法治疗,分析两组头颈部活动度(头部偏斜角度、颈部侧屈活动度和颈部旋转活动度)和中医症候积分。结果治疗前,两组头部偏斜角度、颈部侧屈活动度和颈部旋转活动度比较无差异(P>0.05)。治疗2个月、4个月和6个月后,观察组头部偏斜角度小于对照组,颈部侧屈活动度和颈部旋转活动度大于对照组(P<0.05)。治疗前,两组头面倾斜、眼嘴低下、颈椎侧凸畸形、侧屈受限、旋转受限和面部不对称症候积分比较无差异(P>0.05)。治疗后,观察组头面倾斜、眼嘴低下、颈椎侧凸畸形、侧屈受限、旋转受限和面部不对称症候积分均低于对照组(P<0.05)。结论应用郭氏正骨手法治疗先天性肌性斜颈患儿对于患儿头颈部活动度恢复具有促进意义,且患儿疾病症状将得到有效改善。Objective To analyze the clinical efficacy of treating congenital muscular torticollis with Guo’s Orthopedic Manipulation Technique.Methods A total of 60 children with congenital muscular torticollis treated at the Department of Traditional Chinese Medicine,Putian First Hospital,from April 2021 to April 2022 were selected and randomly divided into a control group(30 cases)and an observation group(30 cases)using a random number table method.The control group received conventional tuina(massage)treatment,while the observation group was treated with Guo’s Orthopedic Manipulation Technique.The cervical range of motion(head tilt angle,lateral flexion range of motion,and rotation range of motion)and traditional Chinese medicine symptom scores were analyzed for both groups.Results Before treatment,there were no significant differences in head tilt angle,lateral flexion range of motion,and rotation range of motion between the two groups(P>0.05).After 2,4,and 6 months of treatment,the head tilt angle in the observation group was smaller than that in the control group,and the lateral flexion range of motion and rotation range of motion were greater than those in the control group(P<0.05).Before treatment,there were no significant differences in symptom scores for head and face tilt,eye and mouth depression,cervical scoliosis,limited lateral flexion,limited rotation,and facial asymmetry between the two groups(P>0.05).After treatment,the symptom scores for head and face tilt,eye and mouth depression,cervical scoliosis,limited lateral flexion,limited rotation,and facial asymmetry in the observation group were all lower than those in the control group(P<0.05).Conclusions The application of Guo’s Orthopedic Manipulation Technique in treating children with congenital muscular torticollis is significant for the recovery of cervical range of motion and effectively improves the symptoms of the disease in children.
