中医针刺联合清肺祛痰汤治疗痰热壅肺型卒中相关性肺炎的效果

摘　　要：目的探究痰热壅肺型卒中相关性肺炎(SAP)患者采用中医针刺联合清肺祛痰汤进行治疗的效果。方法选取2021年1月至2023年6月闽清县总医院收治的痰热壅肺型SAP患者30例,采用随机数字法分为对照组(15例)和观察组(15例)。对照组采用针刺治疗,观察组采用针刺联合清肺祛痰汤治疗,对比炎性指标[中性粒细胞/淋巴细胞比值(NLR)、降钙素原(PCT)和C反应蛋白(CRP)]、细胞免疫指标(CD3^(+)、CD4^(+)和CD4^(+)/CD8^(+))、体液免疫指标(IgA、IgG和IgM)、致病菌清除率和治愈率。结果治疗后,观察组NLR、CRP、PCT水平均下降,低于对照组(P<0.05);观察组CD3^(+)、CD4^(+)和CD4^(+)/CD8^(+)水平均升高,高于对照组(P<0.05);观察组IgA、IgG和IgM水平均升高,高于对照组(P<0.05)。两组致病菌清除率对比无差异(P>0.05)。观察组治愈率为93.33%(14/15),高于对照组的60.00%(9/15)(P<0.05)。治疗后,观察组CPIS评分低于对照组(P<0.05)。结论针对痰热壅肺型SAP患者实施针刺联合清肺祛痰汤治疗的效果显著,有助于缓解炎症问题,增强患者的机体细胞免疫能力,提升治愈率,整体干预效果确切。Objective To explore the effect of treating patients with phlegm-heat obstructing lung type stroke-associated pneumonia(SAP)using a combination of acupuncture and Qingfei Gutan Decoction,a traditional Chinese medicine formula.Methods A total of 30 patients with phlegm-heat obstructing lung type SAP admitted to Minqing County General Hospital from January 2021 to June 2023 were selected and randomly divided into a control group(15 cases)and an observation group(15 cases)using a random number method.The control group received acupuncture treatment,while the observation group received acupuncture combined with Qingfei Gutan Decoction treatment.Inflammatory indicators[neutrophil-to-lymphocyte ratio(NLR),procalcitonin(PCT),and C-reactive protein(CRP)],cellular immune indicators(CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)),humoral immune indicators(IgA,IgG,and IgM),pathogen clearance rate,and cure rate were compared.Results After treatment,the levels of NLR,CRP,and PCT in the observation group decreased and were lower than those in the control group(P<0.05).After treatment,the levels of CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)in the observation group increased and were higher than those in the control group(P<0.05).After treatment,the levels of IgA,IgG,and IgM in the observation group increased and were higher than those in the control group(P<0.05).There was no difference in pathogen clearance rate between the two groups(P>0.05).The cure rate in the observation group was 93.33%(14/15),higher than that in the control group,which was 60.00%(9/15)(P<0.05).After treatment,the CPIS scores in the observation group were lower than those in the control group(P<0.05).Conclusions The treatment of phlegm-heat obstructing lung type SAP patients with acupuncture combined with Qingfei Gutan Decoction is significantly effective,helps to alleviate inflammatory issues,enhance the patient’s cellular immune capacity,increase the cure rate,and the overall intervention effect is definite.
