摘 要:目的探讨五参汤对脓毒性心肌病小鼠心肌损伤的影响。方法小鼠随机分为对照组、模型组、模型+五参汤组,每组20只。除对照组外,其余组均通过腹腔注射脂多糖诱导脓毒性心肌病模型,建模前5 d及建模后7 d各组小鼠给予生理盐水或五参汤灌胃处理(1次/d)。检测3组血清中肿瘤坏死因子-α(TNF-α)、白细胞介素(IL)-18、IL-1β、活性氧(ROS)、丙二醛(MDA)、谷胱甘肽过氧化物酶(GSH-Px)、肌酸激酶(CK-MB)和肌钙蛋白(cTn I)水平;苏木精-伊红(HE)染色观察3组心肌组织病理特征;免疫荧光染色检测3组心肌组织中消皮素D的N端(GSDMD-N)阳性细胞数;蛋白质印迹法检测3组心肌组织中NOD样受体热蛋白结构域3(NLRP3)、半胱氨酸天冬氨酸蛋白酶-1(Caspase-1)、GSDMD-N、IL-18和IL-1β蛋白。结果与对照组相比,模型组、模型+五参汤组小鼠心肌细胞排列紊乱,有大量炎性细胞浸润,血清中TNF-α、IL-18、IL-1β、ROS、MDA、cTnI、CK-MB水平和心肌组织GSDMD-N阳性细胞数及NLRP3、Caspase-1、GSDMD-N、IL-18、IL-1β蛋白升高,血清中GSH-Px水平降低(P<0.05);与模型组相比,模型+五参汤组相对应指标变化趋势与上述相反(P<0.05)。结论五参汤可抑制脓毒性心肌病小鼠炎症反应、氧化应激、心肌细胞焦亡及NLRP3/Caspase-1/GSDMD信号通路活性。Objective To investigate the effect of Wushen Decoction on the myocardial injury in mice with septic cardiomyopathy.Methods Mice were randomly separated into a control group,a model group,and a model+Wushen Decoction group,with 20 mice in each group.Except for the control group,mice in all other groups were injected with lipopolysaccharide to induce septic cardiomyopathy.The mice in each group were given intragastric administration of normal saline or Wushen Decoction accordingly,once a day for 5 d till modeling.And after modeling,the gavage went on for 7 d.The serum levels of tumor necrosis factor-α(TNF-α),interleukin(IL)-18,IL-1β,reactive oxygen species(ROS),malondialdehyde(MDA),glutathione peroxidase(GSH-Px),creatine kinase-isoenzyme(CK-MB),and cardiac troponin(cTnI)were detected.Hematoxylin-eosin(HE)staining was used to observe the pathological changes in myocardial tissue of the three groups.The number of gasdermin D-N terminal(GSDMD-N)positive cells in myocardial tissue was detected by immunofluorescence staining.Western blot was utilized to detect NOD-like receptor family pyrin domain-containing 3(NLRP3),cysteine aspartic protease-1(Caspase-1),GSDMD-N,IL-18,and IL-1βproteins in myocardial tissue.Results Compared with the control group,the model group and model+Wushen Decoction group showed disorderly arranged myocardial cells,a large amount of inflammatory cell infiltration,increased serum levels of TNF-α,IL-18,IL-1β,ROS,MDA,cTnI and CKMB,elevated number of GSDMD-N positive cells in myocardial tissue,increased levels of NLRP3,Caspase-1,GSDMD-N,IL-18,and IL-1βproteins,and decreased serum level of GSH-Px(P<0.05).Compared with the model group,the corresponding indicators in the model+Wushen Decoction group showed opposite trends to those mentioned above(P<0.05).Conclusion Wushen Decoction can inhibit inflammatory response,oxidative stress,pyroptosis of cardiomyocytes,and NLRP3/Caspase-1/GSDMD pathway activity in mice with septic cardiomyopathy.
