摘 要:目的对比高频超声(HFUS)与磁共振成像(MRI)在儿童距腓前韧带运动损伤诊断中的应用。方法选择2023年1-9月四川省骨科医院收治的距腓前韧带损伤患儿99例为研究对象,患儿均进行HFUS和MRI影像学检查,距腓前韧带运动损伤诊断以关节镜检查为金标准,比较MRI与HFUS在儿童距腓前韧带运动损伤中的诊断价值。结果HFUS检出Ⅰ级损伤3例、Ⅱ级损伤38例、Ⅲ级损伤58例,以关节镜检查结果为对照,HFUS诊断距腓前韧带运动损伤Ⅰ、Ⅱ、Ⅲ级损伤的曲线下面积(AUC)分别为0.762、0.781、0.809。MRI检出Ⅰ级损伤2例、Ⅱ级损伤43例、Ⅲ级损伤54例,以关节镜检查结果为对照,MRI诊断距腓前韧带运动损伤Ⅰ、Ⅱ、Ⅲ级损伤的AUC分别为0.927、0.802、0.850。HFUS、MRI诊断距腓前韧带运动损伤Ⅰ、Ⅱ、Ⅲ级损伤AUC比较,差异均无统计学意义(P>0.05)。以关节镜检查结果为对照,HFUS诊断距腓前韧带运动损伤Ⅰ、Ⅱ、Ⅲ级损伤的敏感度分别为66.67%、73.33%、88.24%,特异度分别为98.96%、85.19%、72.92%。MRI诊断距腓前韧带运动损伤Ⅰ、Ⅱ、Ⅲ级损伤的敏感度分别为66.67%、80.00%、88.24%,特异度分别为100.00%、83.33%、81.25%。结论在儿童距腓前韧带运动损伤诊断中,MRI与HFUS均具备较好的诊断效能,二者均可对距腓前韧带运动损伤分级进行诊断。Objective To compare the application of high-frequency ultrasound(HFUS)and magnetic resonance imaging(MRI)in the diagnosis of anterior talofibular ligament sports injury in children.Methods A total of 99 children with anterior talofibular ligament injury admitted to Sichuan Province Orthopaedic Hospital from January to September 2023 were selected as the study subjects.All of them received HFUS and MRI examinations.With arthroscopy results as the golden standard,the diagnostic value of MRI and HFUS in anterior talofibular ligament sports injury was compared.Results Three cases of gradeⅠinjury,38 cases of gradeⅡinjury,and 58 cases of gradeⅢinjury were detected by HFUS.Compared with arthroscopy results,the areas under the curve(AUC)of HFUS for diagnosing gradeⅠ,Ⅱ,andⅢinjuries of the anterior talofibular ligament were 0.762,0.781,and 0.809,respectively.MRI detected 2 cases of grade I injury,43 cases of gradeⅡinjury,and 54 cases of gradeⅢinjury.Compared with arthroscopy results,the AUCs of MRI for diagnosing gradeⅠ,Ⅱ,andⅢinjuries of the anterior talofibular ligament were 0.927,0.802,and 0.850,respectively.There was no statistically significant difference in AUCs between HFUS and MRI for diagnosing gradeⅠ,Ⅱ,andⅢinjuries of the anterior talofibular ligament(P>0.05).Compared with arthroscopy results,the sensitivity and specificity of HFUS for diagnosing gradeⅠ,Ⅱ,and III injuries of the anterior talofibular ligament were 66.67%and 98.96%,73.33%and 85.19%,88.24%and 72.92%,respectively.The sensitivity and specificity of MRI for diagnosing gradeⅠ,Ⅱ,andⅢinjuries of the anterior talofibular ligament were 66.67%and 100.00%,80.00%and 83.33%,88.24%and 81.25%,respectively.Conclusion Both MRI and HFUS are helpful for diagnosing anterior talofibular ligament sports injury in children.Besides,they can be used for grading of anterior talofibular ligament sports injury.
