地诺前列酮栓联合缩宫素在宫颈未成熟足月妊娠产妇引产中的应用效果

摘　　要：目的:观察地诺前列酮栓联合缩宫素在宫颈未成熟足月妊娠产妇引产中的应用效果。方法:选取2023年1月至2024年4月该院收治的76例宫颈未成熟足月妊娠产妇进行前瞻性研究,按随机数字表法将其分为观察组与对照组各38例。对照组予以缩宫素治疗,观察组在对照组基础上联合地诺前列酮栓治疗。比较两组治疗前后宫颈Bishop评分、血清学指标[内皮素-1(ET-1)、白细胞介素-8(IL-8)、前列腺素E_(2)(PGE_(2))]水平,产程时间,以及不良母婴结局发生率。结果:治疗12 h,两组Bishop评分均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05);两组ET-1、IL-8、PGE_(2)水平均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05);观察组第一、第二产程均短于对照组,差异有统计学意义(P<0.05);观察组不良母婴结局发生率低于对照组,差异有统计学意义(P<0.05)。结论:地诺前列酮栓联合缩宫素应用于宫颈未成熟足月妊娠孕妇引产中可提高宫颈Bishop评分和血清学指标水平,缩短产程,以及降低不良母婴结局发生率的效果优于单纯缩宫素治疗。Objective:To observe effects of Dinoprostone suppositories combined with Oxytocin in treatment of cervical immature fullterm pregnancy women with induced labor.Methods:A prospective study was conducted on 76 cases of cervical immature full-term pregnancy women admitted to the hospital from January 2023 to April 2024.According to the random number table method,they were divided into observation group and control group,38 cases in each group.The control group was treated with Oxytocin,while the observation group was treated with Dinoprostone suppositories on the basis of that of the control group.The cervical Bishop score and the serological indexes[endothelin-1(ET-1),interleukin-8(IL-8),prostaglandin E_(2)(PGE_(2))]levels before and after the treatment,the labor time,and the incidence of adverse maternal and infant outcomes were compared between the two groups.Results:After 12 hours of treatment,the Bishop scores of the two groups were higher than those before the treatment,that in the observation group was higher than that in the control group,and the differences were statistically significant(P<0.05).The levels of ET-1,IL-8 and PGE_(2) in the two groups were higher than those before the treatment,those in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The first and second stages of labor in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).Further,the incidence of adverse maternal and infant outcomes in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusions:Dinoprostone suppositories combined with Oxytocin in the treatment of the cervical immature full-term pregnancy women with induced labor can improve the cervical Bishop scores and the serological index levels,shorten the labor process,and reduce the incidence of adverse maternal and infant outcomes.Moreover,it is superior to simple
