瓜蒌薤白半夏汤加减联合生脉饮治疗痰浊痹阻型冠心病心绞痛患者的效果

摘　　要：目的:观察瓜蒌薤白半夏汤加减联合生脉饮治疗痰浊痹阻型冠心病心绞痛患者的效果。方法:选取2019年2月至2023年6月该院收治的62例痰浊痹阻型冠心病心绞痛患者进行前瞻性研究,按照随机数字表法将其分为对照组和观察组各31例。对照组采用生脉饮治疗,观察组在对照组基础上联合瓜蒌薤白半夏汤加减治疗。比较两组临床疗效,治疗前后中医证候积分、心功能指标[左心室射血分数(LVEF)、每搏输出量(SV)、心排出量(CO)]、血脂指标[总胆固醇(TC)、三酰甘油(TG)]、N末端脑钠肽前体(NT-proBNP)和心肌酶谱指标[肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)]水平。结果:两组治疗总有效率比较,差异无统计学意义(P>0.05);治疗后,两组胸痛、神疲乏力、胸闷、面色晦暗等中医证候积分均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);治疗后,两组LVEF、SV、CO水平均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05);治疗后,两组TC、TG水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);治疗后,两组NT-proBNP、CK、CK-MB水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);治疗期间,两组均未发生明显不良反应。结论:瓜蒌薤白半夏汤加减联合生脉饮治疗痰浊痹阻型冠心病心绞痛患者可促进临床症状缓解,增强心功能,降低血脂,但两种治疗方式效果相当。Objective:To observe effects of Gualou Xiebai Banxia decoction combined with Shengmai decoction in treatment of coronary heart disease angina pectoris of phlegm turbidity obstruction type.Methods:A prospective study was conducted on 62 patients with coronary heart disease angina pectoris of phlegm turbidity obstruction type admitted to the hospital from February 2019 to June 2023.According to the random number table method,they were divided into control group and observation group,31 cases in each group.The control group was treated with Shengmai decoction,while the observation group was treated with modified Gualou Xiebai Banxia Tang in addition to the control group.The clinical efficacy,the traditional Chinese medical(TCM)syndrome scores the cardiac function indexes[left ventricular ejection fraction(LVEF),stroke volume(SV),cardiac output(CO)]levels,the blood lipid indexes[total cholesterol(TC),triacylglycerol(TG)]levels,the N-terminal pro-brain natriuretic peptide(NT-proBNP)level,and the myocardial enzyme spectrum indexes[creatine kinase(CK),creatine kinase isoenzyme(CK-MB)]levels before and after the treatment were compared between the two groups.Results:There was no significant difference in the total effective rate between the two groups(P>0.05).After the treatment,the TCM scores of chest pain,fatigue,chest tightness and dull complexion in the two groups were lower than those before the treatment,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After the treatment,the levels of LVEF,SV and CO in the two groups were higher than those before the treatment,those in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).After the treatment,the levels of TC and TG in the two groups were lower than those before the treatment,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Furthe
