摘 要:全髋关节置换术(THA)是治疗髋部骨折的第一选择,能恢复关节功能,显著改善患者生活质量,但因手术创面大、牵涉神经肌肉多,围手术期疼痛管理一直是影响患者预后的重要因素。区域神经阻滞技术的应用不仅能降低静脉麻醉药物对心脑血管的影响,还能减少阿片类药物使用、减轻患者围手术期疼痛、促进关节功能恢复,尤其对高龄患者的预后及转归有积极影响。目前,尚无理想的适配于THA中的神经阻滞方式的统一方案,多种神经阻滞方案均可应用。Total hip arthroplasty(THA)is the first choice for treating hip fractures,which can restore joint function and significantly improve the patients′quality of life.However,due to the large surgical wound and involvement of multiple nerves and muscles,perioperative pain management has always been an important factor affecting the patients′prognosis.The application of regional nerve block technology can not only reduce the impact of intravenous anesthetics on cardiovascular and cerebrovascular diseases,but also reduce the use of opioid drugs,alleviate perioperative pain,promote joint function recovery in the patients,and have a positive impact on the prognosis and outcome of the elderly patients.At present,there is no ideal unified scheme for nerve block methods suitable for THA,and multiple nerve block schemes can be applied.
