摘　　要：急性ST段抬高型心肌梗死作为心血管疾病最严重的危急重症之一,发病率高、死亡率高,是急诊介入医师面临的重大挑战之一。目前主流的急诊介入策略是在急性闭塞血管部位置入药物洗脱支架,从而恢复冠状动脉血流供应,减少心肌梗死范围及改善预后等。但是药物洗脱支架的置入也带来一系列问题,如支架内急性血栓形成、支架内再狭窄,血栓负荷重导致支架置入后无复流等。近年来随着“介入无置入”的理念深入人心,药物涂层球囊的应用越来越广泛。目前药物涂层球囊应用于支架内再狭窄和小血管病变已得到了广泛的认可,但其在急性ST段抬高型心肌梗死介入治疗中的疗效仍研究甚少,本文主要对目前国内外关于药物涂层球囊在急性ST段抬高型心肌梗死介入治疗中的可行性、安全性及有效性等方面加以综述,旨在指导急诊介入策略的选择,改善患者预后。Acute ST-segment elevation myocardial infarction is one of the most serious critical diseases of cardiovascular disease,with high morbidity and mortality,which is one of the major challenges facing emergency interventional physicians.At present,the mainstream strategy of emergency intervention is to insert drug-eluting stents into the site of acute vascular occlusion,so as to restore coronary blood fl ow supply,reduce the scope of myocardial infarction and improve prognosis.However,the implantation of drug-coated stents also brings a series of problems,such as acute stent thrombosis,restenosis in the stent,and no recurrence after stent implantation due to heavy thrombus load.In recent years,with the concept of“leave-nothing-behind strategy”deeply rooted in physicians’hearts,the application of drug-coated balloons is more and more extensive.At present,the application of drug-coated balloon in stent restenosis and small vessel disease has been widely recognized,but its effi cacy in interventional therapy for acute ST-segment elevation myocardial infarction is still little studied.This paper mainly reviews the feasibility,safety and eff ectiveness of drug-coated balloon in interventional therapy for acute ST-segment elevation myocardial infarction at home and abroad.The aim is to guide the choice of emergency intervention strategy and improve the prognosis of patients.
