血清骨钙素、肿瘤坏死因子-α和金属基质蛋白酶-1联合检测对膝关节半月板撕裂患者关节镜下半月板成形术疗效的预测价值

摘　　要：目的探讨血清骨钙素(BGP)、肿瘤坏死因子-α(TNF-α)、金属基质蛋白酶-1(MMP-1)联合检测对膝关节半月板撕裂患者行关节镜下半月板成形术(AMP)疗效的预测价值。方法选择2021年12月至2023年3月就诊于黄河三门峡医院的146例膝关节半月板撕裂患者为研究对象。患者均行AMP,术后3个月根据治疗效果分为有效组(n=125)和无效组(n=21)。比较2组患者临床资料及血清BGP、TNF-α、MMP-1水平,采用logistic回归分析血清BGP、TNF-α、MMP-1水平与患者行AMP疗效的关系,受试者操作特征(ROC)曲线分析血清BGP、TNF-α、MMP-1水平对患者行AMP疗效的预测价值。结果2组患者年龄、术后是否冷敷、术后负重时间、Lysholm评分、国际膝关节文献委员会膝关节评估表(IKDC)评分比较差异有统计学意义(P<0.05);有效组患者血清BGP水平显著高于无效组,血清TNF-α、MMP-1水平显著低于无效组(P<0.05);排除年龄、术后是否冷敷、术后负重时间、Lysholm评分、IKDC评分等影响疗效的因素后,经logistic回归分析发现,血清BGP、TNF-α、MMP-1水平与患者行AMP疗效显著相关(P<0.05);ROC曲线分析结果显示,血清BGP、TNF-α、MMP-1水平联合预测患者行AMP疗效的曲线下面积为0.882,敏感度、特异度分别为85.71%、87.20%,高于各指标单独预测。结论血清BGP、TNF-α、MMP-1水平变化与膝关节半月板撕裂患者行AMP疗效密切相关,可作为临床预测AMP疗效的可靠指标,且三者联合具有较高预测效能。Objective To evaluate the value of combined detection of serum osteocalcin(BGP),tumor necrosis factor-α(TNF-α)and metallomatrix proteinase-1(MMP-1)levels for predicting the curative effect in patients with meniscus tear in knee undergoing arthroscopic meniscoplasty(AMP).Methods A total of 146 patients with meniscus tear in knee admitted to Yellow River Sanmenxia Hospital from December 2021 to March 2023 were selected.All of them underwent AMP and were divided into an effective group(n=125)and an ineffective group(n=21)according to clinical efficacy 3 months after surgery.The clinical data and serum BGP,TNF-αand MMP-1 levels of patients in the two groups were compared.The relationship of serum BGP,TNF-αand MMP-1 levels with the efficacy of AMP was analyzed by logistic regression analysis.The receiver operating characteristic(ROC)curve was used to analyze the predictive value of serum BGP,TNF-αand MMP-1 levels for curative effect in patients undergoing AMP.Results There were significant differences in age,postoperative cold compress,postoperative weight-bearing time,Lysholm score and International Knee Documentation Committee knee evaluation form(IKDC)score between the two groups(P<0.05).The patients in the effective group had significantly higher serum BGP levels,and significantly lower serum TNF-αand MMP-1 levels than that in the ineffective group(P<0.05).After excluding the factors affecting the efficacy such as age,postoperative cold compress,postoperative weight-bearing time,Lysholm score and IKDC score,the logistic regression analysis showed that serum BGP,TNF-αand MMP-1 levels were significantly correlated with the efficacy of AMP(P<0.05).ROC analysis results revealed that the area under the curve(AUC),sensitivity and specificity of combined detection of serum BGP,TNF-αand MMP-1 levels for the prediction of the efficacy of AMP was 0.882,85.71%and 87.20%,respectively.The performance of combined detection of the three indexes was better than that of single index(P<0.05).Conclusion The changes of serum
