摘　　要：目的探讨风险分析指数(risk analysis index,RAI)对大隐静脉曲张手术老年患者预后的影响。方法收集大隐静脉曲张老年患者196例,分为建模组(n=137)和验证组(n=59);建模组根据预后分为预后不良组(n=22)和预后良好组(n=115)。比较患者患肢临床表现-病因学-解剖学-病理生理学(CEAP)临床分级、静脉临床严重程度评分(VCSS)、静脉节段性疾病评分(VSDS)和RAI得分。分析大隐静脉曲张老年患者预后不良的影响因素,以及RAI得分对患者预后不良的预测价值。结果预后良好组和预后不良组CEAP分级比较差异有显著性(P<0.05),预后不良组VCSS评分、VSDS评分和RAI评分均高于预后良好组(P<0.05)。CEAP分级、合并糖尿病、VCSS、VSDS、RAI是大隐静脉曲张老年患者预后不良的影响因素(P<0.05)。建模组RAI和预测模型、验证组RAI和预测模型均对患者预后具有良好的预测意义(P<0.05)。结论RAI评分是大隐静脉曲张老年患者预后不良的影响因素,据此建立的预测模型对患者预后有较好的预测价值。Aim To investigate the influence of risk analysis index(RAI)on the prognosis of elderly patients with varicose great saphenous vein surgery.Methods A total of 196 elderly patients with great saphenous varicose veins were collected and divided into modeling group(n=137)and verification group(n=59).According to the prognosis,the modeling group patients were divided into poor prognosis group(n=22)and good prognosis group(n=115).The clinical manifestations-etiology-anatomy-case physiology(CEAP)clinical classification,venous clinical severity score(VCSS),venous segmental disease score(VSDS)and RAI score of the affected limbs were compared.The influencing factors of poor prognosis in elderly patients with varicose great saphenous vein,and the predictive value of RAI score for poor prognosis,were analyzed.Results There was a significant difference in CEAP grading between the good prognosis group and the poor prognosis group(P<0.05),that the VCSS score,VSDS score,and RAI score in the poor prognosis group were higher than those in the good prognosis group(P<0.05).CEAP grade,diabetes,VCSS,VSDS and RAI were the influencing factors for poor prognosis of elderly patients with varicose great saphenous vein(P<0.05).The modeling group RAI and prediction model,validation group RAI and prediction model all have good predictive significance for patient prognosis(P<0.05).Conclusion RAI score is the influencing factor for poor prognosis in elderly patients with varicose great saphenous vein.The prediction model established on this basis has a good predictive value for the prognosis of patients.
